(Page 4) MINE ID # OR PROSPECTING ID # M-1992-054 INSPECTION DATE 2/25/04 INSPECTOR'S INITIALS RCO The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this inspection. The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS CORRECTION DATE There are no permit ID signs or permit boundary markers, though they are required. 5/15/04 PB or PV: PB CORRECTIVE ACTION: The operator must post an ID sign and install adequate TOPIC(S): SM boundary markers. Proof of such posting and installing must be received at this office _, _, by the date shown at right. 2. The operator is storing an increasing amount of non-mining-related debris at the site, though that is not approved in the mining plan. 4/15/04 PB or PV: PB CORRECTIVE ACTION: The operator must choose one of the options in this report, TOPIC(S): MP and submit a statement to this office committing to implementing it in a timely manner. _, _, This statement must be received at this office by the date shown at right. 3. The pit slope is being allowed to erode, which contributes to increased reclamation cost and deterioration of the topsoil resource. 7/15/04 PB or PV: PB CORRECTIVE ACTION: The operator must eliminate the source of the problem, TOPIC(S): SC described in this report, and fix the damage to the slope. There is a new road on the permit area, leading to an offsite location. 4/15/04 CORRECTIVE ACTION: The use or ownership of the road is not known at this time, as PB or PV: PB discussed in the report. The operator must provide a statement explaining the date of TOPIC(S): OD construction, purpose and ownership of the road. This statement must be received at _, _, this office by the date shown at right. 5. The site contains knapweed (a state-listed noxious weed) and it appears to be spreading to new areas on the site, including some of the topsoil stockpile. 4/15/04 PB or PV: PB CORRECTIVE ACTION: The existing weed control is ineffective.
