(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-ia~3-mi <br />INSPECTION DATE: Iona iarnro <br />INSPECTOR'S INITIALS: pl p <br />This inspection was conducted as a monitoring inspection to evaluate overall pit stability and compliance with the approved <br />mining plan, and to familiarize this inspector with the operation. Mr, Pat Stewart accompanied this inspector on the inspection. <br />A report prepared by Haley and Aldrich, Inc. entitled Geotechnical Addendum - 2001 Annual Report had been received and <br />reviewed by the Division of Minerals and Geology (DMG), and this inspection was also conducted to compare the resultr <br />presented in the report with the conditions observed in the pit. Overall the report indicates that the pit is stable and that the <br />observational method for determining the stability of the benches is functioning well. The DMG agrees that at the time of <br />inspection the method appears to be working well. The slope failure area survey information was compared with the earliest <br />survey, and indicates that no appreciable movement has taken place since the establishment of survey monuments. <br />This inspection began at the site of the failure, (on the east bench) and observed that the pit slope in the area of the failure <br />appears stable. The rock has been subject to weathering and exhibits some open fractures and loose rock in localized areas <br />but is generally stable. From this location, the west side of the central pit was observed to be reasonably stable. The west <br />side has been pre-split, backfilled, and revegetated. Small scarps are observed in the upper portions of the backfill, and are <br />likely indicative of settling, not instability. Several intrusions were observed on the west side to trend almost directly into the <br />hillside, and therefore do not reduce the stability of the slope. The revegetation appears established and when the trees <br />become larger, the slope will begin to blend into the surroundings. <br />The central pit is mined to final depth and is filling with water as a storage facility, and the pipeline is installed. Work has not <br />begun on the south pit. <br />The north end of the north pit was examined and is currently being backfilled and will be revegetated in the near future. <br />Intermediate benches in the north pit were observed to be reasonably stable, with some areas of broken ground in evidence. <br />These broken areas are being monitored by both the operator and Haley and Aldrich and the bench and highwall modified <br />accordingly. Areas that are mined to final highwall are being pre-split blasted to aid in the stability of the highwall. <br />A large engineered fill is planned in the near future in the southwest portion of the north quarry. The operator is reminded <br />that the DMG is to be notified prior to completion of the underdrains so the DMG may inspect their construction. <br />The boundaries appear to be well marked and the operation is conducted within these permitted boundaries. Revegetation <br />is being conducted concurrently with mining, and looks good. Oil and fuel are contained in a suitable facility with secondary <br />containment.
