(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2001-054 <br />INSPECTION DATE 915/07 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This partial inspection was performed by the Division as part of its monitoring of Construction Materials 110 permits, and in <br />response to the operator's incomplete request for bond release. (The release request was not sent by certified mail, though <br />required by Rule 4.17, so it is therefore regarded as incomplete.) The inspection was scheduled so that the operator could be <br />present during the inspection. <br />The required permit ID sign and boundary markers were posted. <br />The channel crossing at the top of the entrance ramp has been repaired with new culverts, and the sediment channel has been <br />partially filled (again) with fine sediments. The operator has performed the maintenance to the pond and spillway, to ensure <br />that it continues to function as designed. This acts to correct a problem noted in a previous report. <br />Most of the permit area excavation has been completed, and much of the reclamation earthwork and grading have been <br />completed, though it is becoming apparent that long-term stability may a difficult condition to attain. There were several <br />examples of this observed during this inspection, such as: The runoff from upgradient carries a large load of coarse sediment, <br />which regularly fills the channel, requiring constant maintenance. The channel contained about 1000 cubic yards of newly- <br />deposited aggregate, which the operator had not yet removed. The upper bench contained another 1000 cubic yards ofnewly- <br />mucked aggregate in a stockpile that had yet to be processed or removed. The pit benches are lower in elevation than the <br />adjacent constructed channel, which will allow runoff to enter the pit. The slopes exhibit several separate scars of erosion or <br />failure that the operator has mended, but all of them occurred in 2007. The natural, uncontrolled meandering of Bridal Veil <br />Creek upstream from the permitted area will eventually cause it to enterthe pit and degrade the slopes and benches, and leave <br />the site without passing through the sediment pond. There is no present means to provide channel control on lands uphill from <br />the site. The operator may need to consult with an engineer to determine what must be done to ensure stability. <br />There are still parts and debris, broken equipment and stockpiles of gravel and soil onsite, which are not approved in the <br />reclamation plan. These items will need to be removed, and in the case of the soil, be spread onsite for reclamation use. The <br />slopes are required to be revegetated according to the approved reclamation plan, though little vegetative growth was observed <br />on the slopes. The site will need to be reseeded until sufficientvegetation has become established. The reclamation items that <br />are lacking were discussed onsite, and the Division's determination that release could not be approved was subsequently <br />discussed further by email after the inspection; the operator has withdrawn the release request. <br />The lower bench of the site, where much of the onsite runoff reports, is not bermed to contain or control stormwater. <br />Numerous locations along the outer margin of the bench exhibit small erosional features where runoff has drained from the <br />site, without benefit of a sediment stilling basin. The operator stated that he has no stormwater management permit. The <br />date. <br />No further items were observed during the inspection.
