.i - <br /> <br />(Page 2) <br /> <br />MINE ]:D # OR PROSPECTING ID # M-94-117 <br />INSPE('T ION DATE 05/20/99 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during a follow-up inspection of the site, confirming <br />the completion of several tasks required after the last inspection. <br />1) Gates and fencing for the site, preventing access from animals and unauthorized members <br />of the=_ public, are locked and in place. <br />2) The diversion trench above the pond, to the west, is clear and has been kept free of <br />sediment. Hay bales have been installed to prevent erosion on the north side and south <br />side. <br />3) A layer of fill has been placed and compacted on the downstream face of the dam, <br />minimizing erosion to the greatest possible extent at this time. However, the silt fence <br />below the downstream face has collapsed again, and the loose rocks on the face of the dam <br />do not appear to be stable. Some form of temporary erosion control is required here, as <br />well as on the topsoil stockpiles, which should be seeded. (PHly <br />4) Piping still exists to transfer make-up water from the Hazel A to the mill. The recent <br />ruins have filled the pond with stormwater runoff, so that it is at least a few feet <br />higher than it was during the last inspection. Options were discussed regarding emergency <br />land application of the water in the pond. Upon reviewing the file, it is apparent that <br />spray application is an acceptable response to high volumes in the pond, but that sampling <br />acid testing of the water sprayed has always been required. <br />Since the joint venture that was planned for [his site has fallen through, the Operator is <br />con t~amplating going into Temporary Cessation and enacting temporary reclamation measures in <br />order to stabilize the site. Several new plans for final reclamation of the site were <br />discussed during this inspection, including seeding of the ore piles still present on the <br />west side of the site, the re-contouring of the permit area to control runoff in the <br />immediate future, and the possible revisions necessary to keep the site maintained and able <br />to re-open. The Operator intends to submit new proposals by late July, when ITEC's financial <br />resources will become available to finance reclamation work. <br />If a Temporary Cessation is requested, DMG would require the notice of temporary cessation <br />address maintenance of the pond and dam, along with an emergency management plan for high <br />pond. levels at a minimum. This would include inspection frequency by site personnel, the <br />response activities at different levels of the pond, and pond stability maintenance <br />activities. This pond was not designed as a permanent impoundment, nor as a holding pond for <br />stor~mwater. Sufficient precautions must be taken if it is planned for those purposes, even <br />in the short term. <br />No other problems were noted at this time. <br />I & E Contact Address cc: Carl B.
