(Page 2)
MINE ID # OR PROSPECTING ID # M-89-004
INSPECTION DATE 5/5/98 INSPECTOR'S INITIALS AJW
OBSERVATIONS
This was a r..ormal monitoring inspection conducted by Tony Waldron of DMG along with Walt
Isenbart of I:it Carson County. The site is located approximately 2 miles west of Siebert on
a tributary :ranch of the South Fork of the Republican River.
The mine identification sign was properly posted at the mine entrance and contained the
required information. The boundaries of the affected area are delineated by the creek banks
along the no::th and south, a county road on the west, and a stock fence on the east. This
operation is typical of many in this part of eastern Colorado in that the material is being
removed from the active channel bottom where the gravel resource is located. Reclamation
will be to grade out the channel area and allow future runoff events to reshape the channel
with alluvia:- deposits. There were no problems noted at this location.
I & E Contact Address
NAME Walt Isenbart
OPERATOR Kit Carson County
STREET P.O.
