(Page 4) <br />MINE ID # OR PROSPECTING ID # M-1998-104 <br />INSPECTION DATE 9/26/05 INSPECTOR'S INITIALS ESC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLSN3/P033IHL8 VIOLATION3 <br />AND <br />CORRHCTIVS ACTION3 <br />1.'~ PROBLEM: <br />Permit boundary markers were noted on site with the exception to the <br />boundary markers to the west. Rule 3.1.12(2) requires that the boundaries PB or PV:PB <br />of the affected area to be marked by markers that are clearly visible and TOPIC(S):SN <br />adequate to delineate such boundaries. October 31, 2005 <br />CORRECTIVE ACTION: <br />The operator will need to post permit boundary markers to delineate the <br />western boundary area. Photographs of the boundary markers demonstrating <br />compliance must be submitted by October 31, 2005. <br />2 .'~ PROBLEM: <br />The Templeton Ditch was relocated along the north permit boundary area pg or PV:PH <br />to facilitate the mining operation. The ditch was rerouted from the TOPZC(S):NP <br />northwest corner to the location of the entrance road. The relocation October 31, 2005 <br />of the ditch was not approved in the mining and reclamation plan. <br />CORRECTIVE ACTION: <br />The operator will be required to file a revision to the permit to <br />address the relocation of the Templeton Ditch. The revision must <br />contain a description of the rerouted ditch, revised maps (mine plan <br />and reclamation plan maps), a design of the relocated ditch, and <br />notarized agreements from the owner(s) of the ditch consenting to the <br />rerouting of the ditch. The revision must be submitted by October 31, <br />2005. <br />3•~ PROBLEM: <br />Topsoil and overburden piles were noted along the perimeter of the permit PB or PV:PH <br />area. The piles will need to be stabilized to prevent wind and water TOP1C(S):TS <br />erosion in accordance with Rule 3.1.9. October 31, 2005 <br />CORRECTIVE ACTION: <br />The operator will need to file a revision to the permit to address <br />stabilization of the topsoil and overburden piles. The revision must be <br />submitted by October 31, 2005. <br />PROBLEM: <br />On the north aide of the pit, mining related debris has been placed in PB or PV :PB <br />the bottom of the pit. The material consists of trees, brush, screens, TOPIC(S):HO <br />conveyors, cable, oil drums other waste items. This material is not October 31, 2005 <br />considered inert ae defined by Rule 1.1(20). <br />CORRECTIVE ACTION: <br />The operator will be required to remove the non-inert material from the <br />pit area by October 31, 2005. If the operator intends to backf ill <br />inert material in the pit (concrete or asphalt fragments), Rule <br />3.1.5(9) requires the operator to notify the Division in writing if <br />inert structural fill generated outside of the approved permit area <br />will be used to backf ill an excavated pit. Until such notice is <br />received the operator may not backf ill inert material in the pit. <br />
