(Page 2) <br />MINE ID ~ OR PROSPECTING ID ~ M-88-090 <br />INSPECTION DATE 11/20/95 INSPECTOR'S INITIALS CLK <br />OBSERVATIONS <br />The following observations were made during a routine Succession of operator inspection. <br />1. TWK has mined outside the permit area, along the western boundary--approximately 1.5 acres <br />have been disturbed. Thia ie a possible violation of the permit, and an amendment or <br />conversion to a 112 may be required to correct these conditions. <br />2. TWK is storing topsoil outside the permit area (see map). This stockpile should be moved <br />within the permit boudariea ae soon as possible or the permit revised to include the <br />stockpile within the permit boundaries. <br />3. TWK Enterprises is storing concrete onaite; this modification to the operations needs to <br />be covered under a technical revision, and an increase to the bond for burial or removal of <br />concrete onaite may be required. <br />4. There appears to be enough topsoil to completely cover the 8 disturbed scree onaite--- <br />approximately 5500 CY. Thia ie slightly leas than the amount required by the permit; soil <br />amendments may be necessary if revegetative efforts suffer due to a lack of topsoil. <br />5. The amount of financial warranty currently posted may be inadequate for the amount of <br />disturbance at the site. A recalculation of the bond will be sent under separate cover within <br />60 days. <br />It may be possible to move the permit boundaries to accommodate the off-site disturbance, <br />since approximately 1.5 acres along the southern boundary were never mined. However, the <br />topsoil storage and disturbance along the western boundary are still possible violations of <br />the permit agreement, and the operator must appear before the Mined Land Reclamation Board <br />at their hearings in January.
