<br />This inspection was conducted by Wally Erickson from the DMG Durango office, 484 Turner Drive, Building
<br />F, Suite 101 ,Durango, CO 81301 ,phone (970) 247-51 93. Glen Williams and Robert Schwind accompanied
<br />the DMG inspector throughout the inspection. The C-JD-6 Mine is located approximately 10 miles west of
<br />Naturita, on the north flank of Monogram Mesa and southern portion of Paradox Valley, at approximately
<br />6000' elevation. Portal, portal bench, waste rock stockpile, roads and vent hole are located in portions of
<br />the S 1 /2 of the SW 1 /4, and portions of the SW 1 /4 of the SE 1 /4, and portions of the SW 1 /4 of the NE
<br />1/4, Section 22, T46N, R17W, NMPM, Montrose County. Additional vent holes and access roads are
<br />located in portions of the SW 1 /4 of the SW 1 /4, Section 21 , and in portions of the NW 1 /4 of the NE 1 /4,
<br />Section 28, T46N, R17W, NMPM, Montrose County. DMG holds $2,500 financial warranty.
<br />Records -Original permit application was submitted 10/7/77 for 2.24 acres affected area. Operator
<br />requested to increase the affected area to 6.24 acres, in adequacy response letter dated 1 1 /18/77. Permit
<br />was approved on 1 1 /29/77; however, the permit was not issued until 7/31 /79. Permit document and DMG
<br />records indicate affected area of 2.24 acres. DMG is requesting clarification from the operator regarding
<br />affected area acreage.
<br />Prior to permit issuance, DMG approved a Technical Revision ITRI to the original permit application on
<br />3/22/78, addressing relocated vent hole. On 7/17/80, operations went into Temporary Cessation ITCI, due
<br />to declining market conditions. On 8/22/85, DMG approved 5-year extension of TC status. On 6/25/90,
<br />DMG approved TR-01, revising mine status from TC to intermittently active. DMG records do not indicate
<br />additional modifications to the permit.
<br />DMG records contain Stormwater Management Plan, revised 10/4/93, for Stormwater Permit No. COR-
<br />040024, with WOCD. Stormwater Management Plan identifies sediment containment basin for runoff from
<br />access road and portions of the portal bench area, containment berm along the rim of the waste rack/low
<br />grade ore stockpile, and silt fence at various locations along the toe of the portal bench and stockpile area.
<br />Signs & Markers -Mine identification sign and affected area perimeter markers were observed. Public access
<br />to permit area is controlled by locked gate. Mine access road was well maintained.
<br />Facilities -Access to the C-JD-6 Mine is provided by the Duggan Adit. Duggan Adit also provides access
<br />to Mineral Joe Claims, permit no. M-77-284. Mine portal was open, rail lines extended from portal to ore
<br />crib. Ore crib and loading areas were clean, no ore observed. Sediment containment berm, along the rim
<br />of the low grade ore/waste rock stockpile, was intact and well maintained. Silt fence, located at various
<br />locations along the toe of the stockpile was intact and well maintained. Sediment containment basin, located
<br />immediately uphill of the stockpile area, was intact and well maintained. Impounded water was not observed
<br />in catchment basin or on top of the stockpile. Erosion/sedimentation within the affected areas and to the
<br />surrounding areas appeared to be controlled appropriately.
