<br />• (Page 2) • <br />MI N~ ID # OR PROSPECTING ID #M-82-044 <br />INSPECTION DATE 12-28-99 INSPECTOR'S INITIALS BK <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 12-28-99. The site is a small <br />110(2) sand and gravel operation located 3 miles Hast of Divide Colorado. The site was <br />originally permitted for maximum five acres of disturbance. In 1993, the operator applied for <br />an amendment to expand the permit area by two acres for a total of seven acres. Present <br />during the inspection was, Mr. Jerry Johnston the operator. There was approximately 2 to 5 <br />inches of snow on some parts of the permit area at the time of the inspection. Site was not <br />active at the time of the inspection. The entire permit area was walked and inspected . <br />Permit boundary markers were properly posted around the entire permit perimeter. The operator <br />agreed to place additional boundary markers along the North Weat portion of the permit area. <br />Since the entire seven acres appeared to have been disturbed the additional markers will <br />allow sub- contractors which the operator allows to mine the site from time to time to <br />properly identify the permit boundary. Along the north west portion of the pit area, the <br />operator had cut a trench to allow surface flow onto the drainage as part of the storm water <br />control. There was no flow at the time of the inspection. Since it is part of the storm water <br />management plan, the Division suggested to place a couple of additional check dams to <br />decrease the erosion potential of the trench. The financial warranty for the Bite was <br />calculated at $7500.00 at the time the amendment was approved it appears adequate at this <br />time. Permit sign was properly posted at the mine entrance. <br />After the inspection, the operator inquired about what it would take to expand the permit <br />area to 9.9 acres, the maximum allowed under a 110(2) permit and the requirements for a <br />succession of operation. Mr Johnston was notified that it would require an amendment which <br />has a fee of $575 for the amendment to expand the permit and that the succession of operator <br />does not require a fee.
