.~ <br />(Page 4) <br />MINE ID # OR PROSPECTING ID # M77-044 <br />INSPECTION DATE 09/13/94 INSPECTOR'S INITIALS RCO <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />1. There are not sufficient records and reports for this 11/15/94 <br />permit, regarding level of operations, water <br />impoundment, and map of locations of activities and PB or PV: PB <br />stockpiles oneite. TOPIC(S): AR <br />Operator will contact the Division of Water Resources, <br />at the address on page 3, regarding the impounded water <br />in the pit, and send a copy of said correspondence to <br />this office (Div of Minerals and Geology), by the date <br />shown. Operator will also send the other information <br />noted above, to this Division by the date shown. <br />2. There are numerous unstable highwalle, adjacent to the 11/15/96 <br />structures along the permit boundary, and in a position <br />of advancing beyond the permit boundary in the near PB or PV: PB <br />future. TOPIC(S): S6 <br />operator will address all noted problem areas, to <br />stabilize the highwalle, protect the structures and <br />stay within the permit boundary. Proof of completing <br />all needed activities will be sent to this Division by <br />the date shown. <br />3. The topsoil stockpiles observed during the inspection, 11/15/94 <br />were not protected from deterioration, and insufficient <br />in volume for the adequate reclamation of the site. PB or PV: PB <br />TOPIC(S): TS <br />Operator will provide a report as to the estimated <br />volumes of topsoil present in the several piles oneite, <br />provide a map of the site indicating the locations and <br />sizes of piles, and provide a proposal ae to how an <br />adequate amount of topsoil will be either obtained <br />oneite during the future development, or imported from <br />off site, in amounts sufficient for successful <br />reclamation according to the approved plan. operator <br />will also commit to protecting existing stockpiles of <br />soil, following the recommendations of the SCS for <br />temporary seeding. Operator will send this information <br />to the Division by the date shown. <br />
