The following problems (PB) and/or possible violations (PV) (and suggested
corrective actions) were identified during this inspection. The problems
should be corrected by the dates given, or they will become possible
violations. The possible violations should be corrected by the dates given
to reduce their severity when considered by the Mined Land Reclamation Board.
The inspector noted on the previous page should be notified of all corrective
actions taken.

1. The berm along the ditch structure has been breached, 9/15/98
causing downhill erosion. (This is a pre-existing
problem.) PB or PV:PB

The operator must repair the berm to protect areas
offsite from further erosion and sedimentation, to the
landowner's satisfaction.
Proof of completion of this remedial work must be
submitted to this office by the date shown at right.
2. The mining in the east pit extends deeper than the 12/15/98
ditch, proper drainage has not been maintained, the
proper setback has not been maintained to the ditch PB or PV:PB
structure, the sideslopes around the pit are too TOPIC(S):MP
steep, and reclamation has not been begun in a timely
manner in areas where mining has been terminated.
These conditions, clearly defined in the permit file,
have not been followed in the operation of this site.
The operator must perform the needed filling and
grading to correct these conditions, plus perform the
neede clean up and revegetation described in the
narrative of this report.
Proof of completion of these items must be submitted
to this office by the date shown at right.
