ORIGINAL -PUBLIC FILE
(Page 2)
MINE ID # OR PROSPECTING ID #: +U_I9Rn_~ae
INSPECTION DATE: 12/~9ind INSPECTOROS INITIALS: -B~
OBSERVATIONS
The Division conducted a bi- monthly monitoring inspection of the site on
12/29/04. The area had received measurable snow the night before which was
evident on the ground. Present during the inspection was Jeff Campbell of
CC&V. Inspected the Phase I, II, III AND IV leach pad areas and re-corded
the following readings from the High and Low Volume Solution Collection
Transducers.
Phase I High Volume solution collection transducer # 1 read out meter was
not functioning; Jeff said that the electricians want to replace the meters
when they pull out the pumps. Since there are two transducer meters and
piezometer for the high volume, pregnant solution collection area, the fact
that the # 1 high volume transducer is not working does not hinder the fact
that the pregnant pond is functioning as approved by the MLRD BOARD. # 2
was at 48. And # 3 was at 49.1. Piezometer for the pond was at 47.8 feet.
Low volume transducer # 1 was at.24 and #2 was at .63.
Phase II & III High Volume solution collection transducer #4 was recorded
at 37.6 # 5 was at 35.4 and # 6 was at 39.3. Low volume transducer # 1 was
at .14 and # 2 was at .34. Piezometer for the pond was at 39.4 feet.
Phase IV High Volume solution collection transducer # 7 was recorded at
29.9 # 8 was at 29.1 # 9 was at 30.5 and # 10 was at 30.8. Low volume
solution collection transducer # 1 was at 12.7 and # 2 was at 8.0 inches of
hydrostatic head. (All hydrostatic head readings are in feet unless
otherwise noted)
Inspected the external pond, it was holding a partially frozen water. While
in the area recorded the following readings from the low volume solution
collection transducers. # 1 was at 12.1 and # 2 was at 10.7 inches of hydro
static head.
Inspected the under drain area, all surface water diversion structures in
the area were in good repair. Both the south under drain and flinch pipe
were discharging.
I & E Contact Address
NAME: MR PHIL BARNES Harry Posey (e-mail)
cc: I' CE
OPERATOR: CC&V I' BL
STREET: P n RnX 191 t FS
I' H W
CITY/STATE/ZIP: yICTnR cCl ~aAnC Rna~n ~ HMWMD (CH)

t SE
t WQCD (CH)
CC: I' OTHER
