Locations as of 15 May 2007 (Environmental Office) <br />Two Environmental Management Documents (EMD) binders and Volumes of the Permit <br />Application Package are in a bookshelf along the north wall. A 4-drawer file cabinet, <br />located in the middle of the room, drawers (3 each) labeled C-1982-056, contain portions of <br />the files. <br />1) A copy of the permit is filed in the Environmental Management Documents (EMD) binder, <br />Tab 2. It expires May 23, 2008. <br />2) The permit application w/revisions is on shelves in the south wall bookshelves. It consists of <br />20 Volumes: I, IA, II (and sub volumes for Exhibits 1 through 49), III (Figures), IV and IVA (Tables <br />1 through 69), Volume V (Maps 1 through 41 A), and Fish Creek tipple Volumes VII, VIII, and <br />IX. <br />3) The Findings Document for the latest Permit Renewal is filed in the EMD (Tab 2) The current <br />one is for Renewal 4 and is dated December 30, 2003. <br />4) The insurance certificate is filed in a file cabinet drawer labeled C-82-056. Certificates <br />expire on 10/O1 of the applicable year. <br />5) Copies of bond documents are in the binder titled Bond Information. <br />b) Air emissions (construction) permit #93R01204 issued April 10, 2001 is on file in the EMD, Tab <br />1. It has no expiration date. <br />7) A copy of the Routt County Special Use Permit (SUP) 00-344, changed to SUP 93-233 valid <br />until September 1, 2003 and extended to 07-1 1-10, is in the EMD at Tab 5. <br />8) UG Mining Landowner Notifications are filed in the 82-056 drawer, in a file labeled "Land <br />Ownership Notifications". <br />9) Subsidence Monitoring Reports are filed in the 82-056 drawer, in a file labeled "Current <br />Subsidence Reports". <br />10) Subsidence monitoring data is stored in a binder, located in the north shelves. <br />1 1) There are no variances on this permit with regard to Rules 2.06.2(1) (Experimental <br />Practices Mining), 2.06.3(4) (Mountaintop Removal Mining, or 2.06.5(3) (Steep Slope <br />Mining). Variances were provided in a previous permit PAP under Rule 2.06.7(5) for Delay <br />in Contemporaneous Reclamation of past surface mining activities which <br />accommodate current underground entries and surface support activities. These <br />variances need to be collated into the current PAP. <br />12) Annual Reclamation Reports are due by February 15 of the following year [Rule <br />2.04.13[ 1) (no other such date agreed upon]].
