(Page 2)
MINE ID # OR PROSPECTING ID #: nn_~nnn_> > ~
INSPECTION DATE: pl/no/nx INSPECTOR'S INITIALS: ~A'L
OBSERVATIONS
l . This inspection was conducted as part of the Division of Minerals and Geology's four-year monitoring
program. Present at the inspection were Kate Pickford, DMG, and Blaine Decker and Adelmo Martinez,
SLV Earth Movers, Inc. Weather at the time of the inspection was clear, calm, and approximately 30
degrees.
2. A sign is posted at the entrance to the site and the permit boundaries are marked with fences. There is
presently no active mining occurring at the site. Approximately 15 acres of the site have been disturbed
by the operation. The last mining activity occurred in 2003. Although no future mining is scheduled,
some reclamation is to be conducted at the site in the near future.
3. The site consists of the mined area, which is a pit with benched walls sloped at approximately 3:1, and
numerous stockpiles of finished material and overburden. Little topsoil was present at the site at the
commencement of mining. The topsoil that could be salvaged, however, has been stockpiled in the
form of berms surrounding the site.
4. All activity at the site appears to comply with the approved plan contained in the permit. A bond
calculation will be conducted separately from this report to determine whether the current bond
amount of $68,865.00 is adequate for reclamation of the site.
I & E Contact Address
NAME: Jim Willschau
OPERATOR: J and K Limestone Products.
