<br /> <br /> <br />~ MINE ID # OR PROSPECTING ID #M-60-244 <br />INSPECTION DATE 11-1-99 <br />(Page 2) <br /> <br />INSPECTOR'S INITIALS BK <br />The Division conducted a monitoring inspection of the site in response to a telephone call <br />received from the operator regarding a Cyanide spill incident outside the protected area. On <br />November 1, 1999, at 6:45am, the Division received a call from Mr Scott Lewis Manager <br />Environmental Affairs about a cyanide spill outside the protected area. According to Mr. <br />Lewis, on Sunday the 31st of October, around 12:OOam, cyanide spilled outside the protected <br />area. He stated that once the spill was noted during a routine inspection, all the necessary <br />emergency response plans were implemented. These include the shutting ad re-routing of the <br />solution to other parts of the leach pad, quick removal and placement of all contaminated <br />soil on to the leach pad and detoxification of the contaminated soil outside of the protected <br />area with Calcium HYPOCHLORITE. The Division arrived on site around 11:OOam and proceeded to <br />the spill area accompanied with Mr. Phil Barnes of CC&V. According to Mr.Barnes, the spill <br />occurred due to what appeared to be a defective reducer pipe which carries in-richment <br />solution from phase I to phase II leach pad area. He stated that during a routine inspection <br />of the pad, an operator discovered a leak from the reducer pipe located south of the ARD <br />building. Once the leak was noted, the pipe was isolated shut off and rerouted using a <br />valve to other parts of the leach pad. He sated that approximately 4o square yards of area <br />outside of the containment was contaminated. He also stated that due to the cold weather, the <br />spill that did not freeze did not advance beyond the adjacent diversion ditch. All the <br />contaminated soil was removed and placed on to the leach pad. The area where the <br />contamination occurred outside the protected area was detoxified using Calcium Hypochlorite. <br />The area of spill was walked and inspected it appears as described by Both Mr. Lewis and Mr. <br />BARNES. Since the spill never threatened the waters of the United States, no samples were <br />collected from the Surface and Ground water Monitoring locations for the site. It is the <br />understanding of the Division that the operator had sampled the soil in the contaminated area <br />after detoxification.
