ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_tean_~ee <br />INSPECTION DATE: ~-~e_nd INSPECTOROS INITIALS: -BK- <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 2-24-04. The <br />main purpose of the inspection was to sit down with CC&V and try to resolve <br />some of the outstanding LAOIS in file with the Division. The Division had <br />sent letters to CC&V detailing the outstanding LAOIS, that could have <br />duplicate coverage, some that have already been reclaimed, etc. etc. Prior <br />to inspecting the Phase II pregnant solution collection area, I sat down <br />with Mr. Phil Barnes and went over in detail over the contents of the <br />letters, and what it would take to remove the old LAOIS (Notice of Intent). <br />CC&V has in place a State Wide Financial warranty in the amount of <br />$300,000.The Division and CC&V wants that warranty to cover all prospecting <br />both current and future in the mining and geologic districts with in the <br />State. The bond is currently attached to Prospect permit # P 1999-015.A11 <br />the old LAOIS are with in the permit area, with the exception of a few, and <br />the ones with in the permit area are already covered under the financial <br />warranty for permit # M-1980-244's affected acreage. Mr. Barnes agreed to <br />provide a response to eliminate the duplication and remove, the old LAOIS <br />from the Division's data files. <br />After the meeting, inspected the Phase II pregnant solution collection area <br />and recorded the following readings from the High and Low Volume solution <br />collection system transducers. <br />High Volume Solution Collection Transducer # 4 was recorded at 41.4, # 5 <br />was recorded at 43.0 and #6 was at 43.0. Low volume transducer # 1 was <br />recorded at 1.03 # 2 was at .06, (all readings are in feet of hydrostatic <br />head.) No major problem noted. <br />I & E Contact Address <br />NAME: MR.
