The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />1. The mining and reclamation permit is not current with the site <br />conditions and the Construction Material Rules and Regulations <br />(Rules 6.4.4 & 6.4.5). <br />CORRECTIVE ACTION: <br />The operator will need to update the mining permit with the <br />revised mine plan, include the asphalt batch plant in the <br />permit, and a plan to achieve the required slope configuration <br />noted on page 3 of the inspection report. The due date for the <br />technical revision is May 4, 1999. <br />2. Permit boundary markers were not located on site. <br />CORRECTIVE ACTION: <br />DATE <br />PB or PV:PH <br />TOPIC(S):RS <br />PB or PV:PB <br />TOPIC (S):SM <br />The operator will need to post permit boundary markers as <br />required by Rule 3.1.12, and submit photographs of the markers <br />to the Division by May 4, 1999 as proof of compliance. <br />
