MINE ID # OR PROSPECTING ID #: M-~ 9RV-nnn <br />INSPECTION DATE: (L/2binv INSPECTOR' S INITIALS: KAP <br />OBSERVATIONS <br />1. This inspection was performed as part of the Division of Reclamation, Mining, and Safety's four-year <br />monitoring program. Present at the inspection was Kate Pickford, DRMS. Weather at the time of the <br />inspection was clear, calm, and approximately 75 degrees. <br />2. During the attempts to contact the operator to notify him of the inspection, it was discovered that the <br />Division does not have a current, working phone number for the operator. The Operator should supply a <br />current, working contact number to the Division. <br />3. A sign is posted at the entrance to the site and the boundaries are easily identifiable in the field. The <br />disturbance at this site consists of a large excavated pit with stockpiles on the pit floor. Vertical <br />highwalls exist along the north and west sides of the pit. The north highwall is oriented only about 10 <br />feet from the north boundary fence. This will affect the amount of bond necessary for reclamation of <br />the site, in that some backfilling will be required to attain the final 3H:1 V slopes. <br />4. There are a large number of various items and equipment stored on the pit floor, many of which appear <br />not to be affiliated with the mining activity at the site. These items include: <br />• Pieces of heavy equipment <br />• A mobile home <br />• 55-gallon drums (13) <br />• 5 gallon pails (15) <br />• 3-foot wide rolls of conveyor belt material (4) <br />• Tires.(30) <br />• Trucks <br />• Household garbage <br />5. The presence of these items will affect the amount of bond necessary for final reclamation of the site. <br />Additionally, some of these items at the site are problematic. The contents of the 5-gallon buckets and <br />the 55-gallon drums are unknown. Therefore, unless the contents can be verified as non-hazardous, the <br />drums and buckets need to be removed from the site (See problem 1, page 3 of this report). <br />b. The household garbage present at the site is also a problem, in that the Construction Materials Rules <br />and Regulations specify that any offsite mpterials used as backfill onsite must be documented as inert. <br />(See problem 2, Page 3 of this report). <br />A new bond amount will be calculated for the site.
