MINESITE INSPECTION NARRATIVE 4 <br />tank located off of the permit area between the northwest pond and this <br />one. Apparently there was a spill, the discharge from that area goes <br />through this pond. <br />An attempt was made to cross the upper area to check on the maintenance of <br />diversions and if there were any undrained areas on the reclaimed area. No <br />problems were noted. The diversion on the northeast perimeter above the <br />northwest pond was checked. It appeared to be stable. Perimeter markers <br />were noted as we traversed the mine. Several shrub planting plots were <br />checked as we worked across the slope. There doesn't appear to be much new <br />growth or regeneration in those areas. Revegetation on the areas that were <br />not covered with too much snow appeared to be uniform. The south east <br />facing slopes above pond 2 did not have good growth, as was noted during my <br />last oversign inspection of the mine. These slopes appear to have been <br />reseeded with better results this time. <br />COMMENTS ON RECORD REVIEW <br />*Reclamation only mining permit, C-81-029, expires August 9, 1993. <br />*Their NPDES permit, COG-850000, expires June 30, 1997. <br />*Certificate of Public Liability Insurance, policy # 920033, issued from <br />Flattop insurance Agency in west Viriginia is in effect until November 1, <br />1993. The coverage, the mine is named on the policy, meets the required <br />minimum for comprehensive and general liability. <br />*Cert if ication for "AS Suilt" of the: <br />Explosives Storage Pond July 5, 1990. <br />Yampa River Pond July 10, 1992. <br />Sediment Ponds, Upper 1 5 2, October 1986. <br />*La st reported quarterly self inspection of ponds was completed August 7, <br />1992. <br />*Surety bond on file with the State of Colorado is posted for $307,000.00. <br />*Surface and ground water monitoring plan was updated February 5, 1986. <br />Requirement for monitoring springs and spoil seeps with 3 gpm minimum. <br />Question about whether this has been done. The comment was that none have <br />been reported to date. <br />*Quarterly NPDES discharge records are up-to date with the oil 6 grease <br />self reported violation occurring June 12, 1991- 200 mg\1. <br />CLOSEOUT FOR THE INSPECTION: <br />No outstanding violations. None issued during the inspection. No <br />enforcement action issued from this office. Mss Morrison had a number of <br />concerns that were addressed during the closeout. She also has made some <br />progress towards cleaning up the status of a number of issues on the mine <br />as related to structures and equipment to be removed or left. A November <br />25, 1992, letter was sent to Mz. Eilts regarding 11 of those items. A <br />number of those issues were addressed in the narrative of this report. Her <br />letter to Mr. Eilts indicated that these matters need to be cleared up by <br />the summer season of '93 inorder to identify reclamation still needed. <br />
