ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: nn_IeRn-gee <br />INSPECTION DATE: 9-> >-n'~ INSPECTORDS INITIALS: ___BK.- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the Phase I, II, III AND IV areas of <br />the leach pad and water sampling from the North Under drain Discharge, on 9-11-03. Present during the <br />inspection was, Tim Comer of CC&V. The physical inspection began around the Phase IV pregnant solution <br />collection areas. Only drain cover was being pushed along the west and North sides of the pad area, at the <br />time of the inspection. <br />While in the area collected the following readings from the High and Low Volume Solution Collection <br />Transducers for Phase IV. High Volume Transducer # 1 read out was not functioning at fhe time of the inspection, <br />the pump was on, and as noted in the previous inspection reports, once all the power transfer is complete, it <br />should be functioning as designed. The main power poles are already in place and once the permanent pumps <br />are in place, it is expected to function as designed. # 2 was at 18.6 feet of hydrostatic head. Low Volume <br />transducer # 1 was at 4.0 and # 2 was at 2.5 (all readings are in inches of hydrostatic head) both pumps were on <br />at the time of the inspection. No major problem noted. Inspected the leach pad in the Phase IV PSSA area, very <br />SMALL puddles of solution, AS REPORTED IN THE PREVIOUS INSPECTION REPORT, a D-6 DOZER was ON SITE TO <br />REWORK THE UPPER LAYER, to allow percolation, no major problem noted. <br />Inspected the Phase I AND II Pregnant solution collection areas and recorded the following readings from the <br />High and Low volume solution collection areas. <br />Phase I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS recorded at 49.5# 2 was at 51.4 and # 3 was <br />at 51.8. Low volume transducer # 1 was recorded at .23 # 2 was at ,75. <br />PHASE II HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 37.7 # 2 WAS AT 34.7 AND #6 <br />WAS AT 39.5 LOW VOLUME TRANSDUCER # 1 WAS Ai .17 AND # WAS AT .60 (ALL READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD) Piezometer for the pond was recorded at 34.1 feet. <br />Inspected the external pond, it was holding water from the recent storms, while in the are recorded the following <br />readings from the Low Volume solution collection transducers # 1 was at 7.6 and # 2 was at 8.7 readings are in <br />inches of hydrostatic head. Earth moving work in anticipation of liner DEPLOYMENT IS CONTINUING. Once Poly <br />Flex returns to the site, CC&V expects to have the LINER WORK COMPLETED. <br />Inspected the under drain area. Both the 4-inch pipe, South and North under drains were discharging at the <br />time of the inspection. The flow was being pumped back onto the leach pad.
