(Page 21 <br />MINE ID # OR PROSPECTING ID Jf M-1995-060 <br />INSPECTION DATE 2/26/02 INSPECTOR'S INITIALS RCO <br />OBSERVATIONS <br />This inspection was performed by the Division as part of its monitoring of Constrctuin Materials 112 permits. The operator <br />was notified but was not present during the inspection. <br />The required permit ID sign was posted near the entrance to the permit area. The north boundary is the county road, and <br />the east boundary was marked by an earthen berm a short distance inside the boundary. Disturbance extended along the <br />eastern portion of the area nearly to the SE permit corner. The operator must ensure that no disturbance extends beyond <br />the boundaries, and placement of corner or perimeter markers is the best method. Please ensure that markers are in place. <br />The NE corner is an old pre-permit pit, which does not appear to have been redisturbed. The active excavation is limited <br />to the south east and east central part of the area. An estimate of the extent of the current disturbance was made, with <br />a total of about 11 acres, including the berms, road and new pit. <br />The pit is about 3 or 4 feet deep. Much of the pit floor is rough and deeply rutted by scraper tires. A few frozen puddles <br />and alkali deposits are the only features in the pit. The side slopes vary from vertical to about 2:1 gradients. No stockpiles <br />or equipment are in the pit. Unexcavated areas surrounding the pit, within the permit area, exhibit a lot of old household <br />trash. The operator has committed to removal of the trash from areas to be mined prior to mining. <br />It is not clear if the perimeter berms are topsoil to be replaced. The operator should update the map which is submitted <br />with the annual reports and include a descriptive label on these berms in question. Other updates should include a more <br />current delineation of current areas of disturbance. <br />The bond was calculated a year ago, and is still sufficient to accomplish final reclamation on all disturbance. No <br />recalculation will be performed at this time. <br />There were no additional items observed. No problems exist at this time.
