-:~ <br />III. COMMENTS -COMPLIANCE <br />Below are comments on the inspection. The comments include discussion of observations made <br />during the inspection. Comments also describe any enforcement actions taken during the inspection <br />and the facts or evidence supporting the enforcement action. <br />General Comments <br />This was a partial site inspection conducted by Dan Mathews of the DMG Grand Junction Field Office. The <br />reclaimed Loma Loadout was inspected at 10am, mine site at 10:45am. Weather was clear and warm, all areas <br />of the mine were snow-free and accessible, with generally dry ground conditions. There was snowmelt runoff in <br />East Salt Creek; no flow in Munger Creek. <br />Signs and Markers <br />The Mine I.D. signs at both the mine and loadout need to be replaced or revised to reflect the permittee <br />name change to CAM Mining LLC. <br />Roads <br />Spring maintenance work is warranted along the canyon section of the mine bench access road, to clear <br />rock fall and better define the road ditch. Soil removed from culvert sumps and other road locations during <br />maintenance work should be transported to the portal bench area, and should be stockpiled adjacent to the east <br />side of the existing construction dirt stockpile. <br />The culvert outlet sleeve for the upper road culvert (CMP #1) needs to be replaced promptly. <br />At the third CMP road culvert inlet, the berm adjacent to the inlet sump has been built up recently, and <br />needs to be seeded. <br />At the fourth CMP road culvert, the inlet was partially obstructed by a large grass clump that needs to be <br />removed. Installation of a series of water bars between the third and fourth CMP road culvert inlets is <br />recommended, to direct road drainage to the inside road ditch. <br />At the fifth road culvert, sediment material recently cleaned out of the inlet sump had been placed on top of the <br />road berm. The material needs to be pulled off the berm and hauled to the portal bench stockpile area. <br />The disturbed berm segment will need to be seeded. <br />Water bars installed last fall along the two-track road that provides access to the small topsoil stockpile adjacent <br />to the lower segment of the Munger Canyon haul road have been breached. These water bars need to be <br />restored, and enlarged, as appropriate, to ensure that flow is diverted off the two•track. <br />There is rock and woody debris lodged at the inlet of the 36" road culvert near the waste disposal area <br />pond embankment, slightly constricting the inlet opening. The debris should be removed. <br />The roadside ditch at the southeast corner of the waste disposal area topsoil stockpile needs to be <br />cleaned out, and the berm separating the road ditch from the topsoil stockpile containment ditch needs <br />to be built up. <br />The temporary drill pad access road up the North Fork of Munger Creek appears to have been reclaimed. <br />Merve Heinecke of Extreme Construction, the contractor who performed the work, reported that the work had <br />been completed, with the exception that additional seeding was warranted along the lower section of the road. I <br />walked a short section of the lower end of the road, and the grading looked quite good, with the surface Left in a <br />properly roughened condition, and woody debris pulled back across the disturbed area. The Munger creek <br />channel was properly restored at the crossing location, adjacent to the primary mine road. Operator placed an <br />order for additional seed during the inspection and indicated that the remaining seeding along the road would be <br />3 <br />
