(Page 2)
MINE ID # OR PROSPECTING ID #: M-1980,-007
INSPECTION DATE: n.5/n9mn
OBSERVATIONS

INSPECTOR'S INITIALS: ~I K
The following observations were made during an inspection in response to a complaint
received through the Larimer County Planning Department. It was believed that the
Operator had gone outside the boundaries of the permitted 2.5 acres.
1) The boundaries have been adequately marked with fencing, road boundaries, and
posts, except along the face where the questionable disturbance occurred. An a
review of the reclamation and mining maps led DMG to the conclusion, after seeing
the site, that the permit boundaries for this site may follow the upper ridge
around the mapped area, and do not extend downward toward the creek. Please mark
the boundaries near the disturbance, so we may determine where the limits of the
permit area are. (P81)
2) The possible excess site disturbance appears to be less than 0.5 acres along the
east side of the site, down the hillside from the permitted area. Shale rocks have
slid down the hillside toward the creek, but have been restrained from choking or
entering the creek before this point in time by a system of netting and chicken-
wire that extends across the bottom of the slide area. When the site was inspected
on May 9`°, a 15'long chicken-wire restraint area between two trees had collapsed,
leaving a hole in the restraint netting which allowed rocks to tumble through and
into the creek.
3) Topsoil adequate to finish reclamation has been salvaged and stored at the site.
4) The Operator's representative present at the time of the inspection said that the
problem had been noted by the Operator, and that additional measures were being
taken to prevent any further damage to the creek, and to repair the current
disturbance. These measures were to include railroad ties, a chain-link fence, and
the building up of the rock wall at the bottom of the slope.
DMG wishes to conduct a second follow-up inspection of this site prior June 23, in
order to judge the efficacy of the Operator's reclamation measures as well as more
accurately measure the off-site disturbance.
I & E Contact Address
NAME: Glenn Southwick
OPERATOR: Masonville Excavating
STREET: 220 W .o m)v Rd. 84.
