• . <br />:~ <br /> <br />MINE ID # OR PROSPECTING ID # <br />INSPECTION DATE 12/19/96 <br />(Page 21 <br />M-83-208 <br />OBSERVATIONS <br />M <br />INSPECTOR'S INITIALS WHE <br />This complete inspection was conducted by Wally Erickson from the DMG field office in Durango, <br />484 Turner Drive, Building F, Suite 101, Durango, CO 81301, phone (970) 247-5193. Mr, Timothy <br />Roberts accompanied the DMG inspector throughout the inspection. The Shavano Pit is located <br />approximately 5.5 miles SW of Montrose, 6100' elevation, in portions of the NE 1 /4 of the NW 1 /4 <br />of Section 11, T48N, R10W, NMPM, Montrose County. The county 112 permit was issued on <br />3/7/84 for 18.46 acres affected area. No activity was observed at the mine site during the time <br />of this inspection. <br />Mine identification sign was posted at the location where the mine access road departs the paved <br />county road, approximately 0.25 mile from the permit boundary. Rule 3.1.1211) states that the <br />mine identification sign shall be posted at the mine entrance. Please relocate the sign to the <br />location where the access road crosses the permit boundary, in accordance with Rule 3.1.12(11. <br />Mine identification sign displayed all information required by Rule 3.1.12(1). Affected area <br />perimeter markers were posted. <br />Mine access road was well maintained. Public access is controlled by locked gate and fence. <br />Permit area is bordered on west side by area previously disturbed. Stockpiles of large rock, gravel <br />and earthen material and discarded equipment were located to the west of the permit boundary. <br />Operator indicated that area west of permit boundary was utilized by "Waterusers" for storage of <br />maintenance/repair materials for irrigation ditches and flood control. <br />Topsoil had been salvaged and stockpiled along the east and south sides of the pit. South side <br />stockpile was located inside the south permit boundary. Recently stockpiled topsoil did not have <br />protective vegetative cover. Rule 3.1.9(1) and (31 require the operator to establish protective <br />vegetative cover on topsoil stockpiles which will remain for periods exceeding one 11) year. Please <br />ensure all topsoil stockpiles, to remain in stockpile configuration for 1 year or longer, have <br />protective vegetative cover. <br />Highwalls were visually estimated at 10' height, 2H:1 V slope and appeared stable. <br />Mining operation appeared limited to approved areas. No evidence of erosion/sedimentation was <br />observed within affected areas and to the areas surrounding the permit boundary. <br />I & E Contact Address <br />NAME Timothy L.
