PROBLEMS/POSSIBLE VIOLATIONS AND CORRECTIVE ACTIONS 1 Patch and expand liner as described in observations Tailing Dond liner damaged in numerous locations, existing liner not adequate to provide coverage at dam crest. Armor pond outlet and clean out ditches ae described in the observations section of this report. Trash and equipment scattered around mill site.
