(Page 2) MINE ID # OR PROSPECTING ID # M-94-117 INSPECTION DATE 8/26/95 INSPECTOR'S INITIALS ACS OBSERVATIONS Present for this inspection were Mark Steen, who owns land within a portion of the permit area as well ae adjacent land, and Bob Mason, a resident of Gold Hill, and a representative of the Committee on Hining and Environment within Gold Hill Town Meeting, Inc. Tailing Pond Liner Patches had been applied in several locations on the liner since the last inspection which was conducted on 7/19/95. The patches generally consisted of thin stripe of HDPE placed over damaged areas and liestered {hot air device) to the existing liner. This type of patching ie not acceptable under the terms of the permit. It was observed that the patches could be removed with little or no effort. All liner flaws and defects must be repaired using an extrusion welding apparatus. All repairs must be vacuum tested. The operator moat provide the Division with a set of technical specifications detailing the repair and vacuum testing procedures to be employed. All repairs must be completed by November 1, 1995. It was agreed that new 30 mil HDPE would be welded to the existing liner at the tailing dam crest, such that existing defects will be cut out of the existing liner or covered by the new liner, and such that there is sufficient new liner attached to completely Beal the upstream face of the dam and most of the dam crest, and such that the liner can be adequately anchored or covered on the dam crest. This will be completed by November 1, 1995. Attaching the strip of new HDPE liner at the tailing dam treat will require removal and resetting of the existing fence poste. The fence must be reinstalled in a workman like manner such that the poste are well set, using concrete if necessary, and the fence ie tight and secure. The poets should be.inatalled near the outside edge of the dam crest in order to maximize coverage by the HDPE liner. A major liner flaw that exists is at the top of the back of the tailing pond. A tree has grown up at the edge of the liner at this location. It was agreed that a new piece of 30 mil HDPE would be welded on to the existing liner at this location, and that the liner would be reinstalled in an anchor trench of the dimensions specified in the permit. Storm Water Controls The outlet of the small sediment pond below and between the tailing pond and the mill building is becoming eroded. it was agreed that the operator would armor the pond outlet with rip rap. This must be completed by October 1, 1995. The diversion ditches above the tailing pond are silting in at some locations. The operator agreed to clean out any silt or debris in the ditches that would be an impediment to flow, and to trim the vegetation in the ditches. This must be completed by October 1, 1995. , General Site Condition The operator agreed to clean up all junk and equipment that ie scattered around the site. Any trash must be removed to an off-s?te landfill. Any usable equipment must be consolidated into a single organized bone yard near the mill building. Any chemicals or petroleum products must be stored inside the building; empty barrels must be removed from the site and properly disposed of. Site clean-up must be completed by October 15, 1995. , Oil saturated soil was observed around the compressor and vacuum pump housed in a lean-to at the uphill end of the mill building. This contaminated soil must be excavated and properly disposed of off-site by November 1, 1995. Hazel-A Adit Several small leaks from the Hazel-A bulkhead were observed. There was a small discharge from the Hazel-A adit to Cash Gulch at the time of this inspection. The water level was approximately ZO" below the top of the bulkhead. No water has been pumped from the Hazel-A up to the pond this summer, however, by the time this report is being written (9/5/95) the operator has reported that pumping of Hazel-A water to the pond was initiated August 30, 1995. It was agreed that the operator would install a lined coffer dam and pump-back system outby of the bulkhead to prevent discharge from the Hazel-A. This system must be in place by October 1, 1995. It was agreed that if the coffer dam is not effective in eliminating discharge from the Hazel-A, that a concrete cut-off wall would be installed and keyed into bedrock. If the concrete cut-off ie needed, it would have to be installed and operational by November 1, 1995. It is imperative that all water be purged from the Hazel-A, through
