(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1978-116 <br />INSPECTION DATE 8/23/2004 <br />INSPECTOR'S INITIALS SSS <br />This was apre-operational inspection for areas to be incorporated into the SM-18 Permit area through the pending <br />Amendment No. 1 (AM-O1) process, and a release inspection for those areas to be released from the permit area through the <br />pending Acreage Reduction No. 1 (AR-01) process. <br />Boundaries of the existing disturbances, as identifed on the AM-01 maps, were walked and verified as accurate. Most of these <br />existing disturbances have been reclaimed, including the SM-18 portal, and are in the vegetation establishment phase. The <br />plugged vent hole area was not inspected. <br />Areas proposed to be affected by the original mining plans that were never disturbed were inspected and verified (as much as <br />possible) to be undisturbed by mining activity under this permit. There are many disturbances in the area, such as power line <br />roads and poles, prospecting roads and diggings, and stock pond excavations and access roads. DMG agrees that the <br />disturbed acreage under this permit is much less than the permitted acreage figure and that a swap of 5 undisturbed acres for 5 <br />new acres to be added to the permit is acceptable. <br />The area to be added into the permit was also inspected. The proposed new waste rock pile /ore stockpile location appears to <br />be undisturbed ground covered mostly by grasses and a few shrubs. This area is adjacent to the north side of County Road <br />U-17 and lies just above the old Wright Mine portals that are also to be added to the permit to afford a more'desirable access to <br />the workings where ore reserves still remain. This stockpile area has one swale that cuts through the west end. Plans show <br />that the growth medium stockpile will be located on the west end of the area, on the west side of the swale. The waste rock <br />and product will be stockpiled on the east side of the Swale. The plan proposes to divert upland drainage around the stockpiles <br />with most of the drainage being routed around the east side of the area, or through the Swale location. Sediment control and <br />stormwater drainage from the stockpile area is proposed to be addressed by a berm. Demonstrations will need to be made to <br />show this proposal will handle significant precipitation events. <br />The existing W right mine disturbance includes a revegetated portal bench and bench outslope with an old historic ore bin. The <br />pad is estimated at 100+ feet long by 25 feet wide and lies at the base of a 30 foot vertical sandstone wall through which the <br />portals will access the workings. A ramp road is proposed to be blasted into fhe rock wall from the east end of the bench <br />toward the northwest to access the upper existing pad from the portals. The upper pad is estimated atone-quarter acre and <br />lies on the crest of the sandstone wall above the portals. It is a partially vegetated, mostly level area with some soil covering <br />the sandstone. <br />The proposed vent hole site was also inspected. This area is located above an ofd stope. The location is a partially vegetated <br />area that has been leveled off in the past and has an old drill hole located in it. The existing road lies adjacent to the pad. <br />Electricity will need to be brought to the site to operate the fan. No provisions were noted in the permit area for surface <br />disturbance associated with installation of the electrical service for this fan.
