(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1977-311 <br />INSPECTION DATE: 11-06-06 <br />INSPECTOR'S INITIALS GRM <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Reclamation, Mining and Safety for permitted sites. The SR-13-A Mine is located north of Slick Rock in <br />Southwestern Colorado. The 110(2) permitted site is fully reclaimed and being monitored for revegetation success. <br />Additionally, there is a possibility that the mine could be re-activated at a later date so release of the site is not <br />necessarily in the future. Glen Williams, representative for Cotter Corporation and Ed Cotter representative for the <br />Department of Energy accompanied the inspector on site. <br />The mine identification sign and affected area boundary markers are not marked. This is not an issue as the site is in <br />full reclamation. However, if reactivation occurs, all boundaries and a proper sign will be required prior to any new <br />disturbance on site. <br />As noted above, the site is fully reclaimed. V egetation is doing very well, with primary species being four-winged <br />salt bush, rabbit brush and perennial gasses of Indian ricegrass, squirrel tail, pubescent and crested wheat. Prickly <br />pear cactus was also observed in several pocks on the slopes. The density and variety indicate a well established <br />plant community. <br />The area has had significant record rain fall in the past month. The site has faired wall with some impact from <br />erosion. Impacts are limited to Tilling and gullying with heaviest impacts along the north slope of the old waste <br />piles. The impact appeazs to be from off site flows running onto the reclaimed slopes from the rim rock above. <br />Pocking has helped contain sedimentation and provided additional erosion control. At this time, no off site damage <br />appeazs to have occurred. The erosion in this area should be monitored for further degadation. If it appears that <br />future rain events continue to cut into the piles some repairs to the slopes and a possible diversion structure at the top <br />may have to be constructed. Currently damage is not significant enough to warrant disturbing established vegetation <br />for repairs. <br />A major drainage runs along the south side of the reclaimed mine piles. Close inspection notes that apre-law mine <br />site dump was heavily eroded during the latest events. A soil debris fan was noted below the mine and expands to <br />the toe of the reclaimed slopes. The Fan clearly comes from the old mine site and not from the permitted area. <br />The mine portal is bat gated just inside the entrance. Other emergency and air portals are also grated and secure. <br />The bond was recalculated in 2003. Since the site is unchanged from that date, an update of the reclamation liability <br />will be done using accepted final tasks. <br />No issues or problems are noted at this time. <br />
