(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-t vs;n_~ae <br />INSPECTION DATE: ~-~~-n~ INSPECTOROS INITIALS: ~K <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress inspection of the site on 7-23-03. Present during <br />the Inspection were, Messer Phll Barnes and Mark Lindquist of CCBV. <br />The Physical inspection began at the Phase IV leach pad PSSA area. CC& V HAD PLACE A TEMPORARY LINER <br />SEAL ABOVE LOW VOLUME SOLUTION PIPES TO PREVENT RAIN WATER DISCHARGE FROM ENTERING THE PIPES. This <br />temporary seal will be removed once these pipes are ready to be extended above the pregnant solution <br />collection area. Solution was being applied to the ore loaded on the pregnant solution collection area at the <br />time of the inspection. No major problem noted. Due to recent rainfall in the area, only drain cover was being <br />deployed along the East side of the Phase II ore storage area. Golder QA/ QC was present over seeing the <br />deployment. Bcakflll and grading of the Phase IV C area was continuing, no major problem. While In the area, <br />collected the following readings from the Hlgh and Low volume solution collection transducers. High volume <br />collection transducer #1 was recorded at 19.4 and # 2 was at 19.5. Low volume collection transducer # 1 was <br />recorded at 6.3 and # 2 was at 7.8. (All readings are in inches of hydrostatic head). <br />Inspected the external pond, It was holding water from rainfall, while in the area, collected the following <br />readings from Low Volume solution collection Transducers. # 1 was recorded at 8.9 and # 2 was at 11.5 . <br />(readings are in Inches of Hydrostatic head) <br />Inspected the Phase I AND II pregnant solution collection areas and recorded the following readings from the <br />Phase I AND II HIGH AND LOW VOLUME SOLUTION COLLECTION TRANSDUCERS. <br />Phase I Hlgh Volume solution collection transducer # I was recorded at 39.8 #2 was at 43.2 and # 3 was at 42.1 <br />Low volume solution collection transducer # 1 was at .21 and # 2 was at .53. Piezometer for the area was <br />recorded at 45.3 feet. <br />Phase II High volume solution collection transducer #4 was at 25.8 #5 was at 20.8 and # 6 was at 25.9 <br />Low volume solution collection transducer #1 was at .13 and # 2 was at .37. Piezometer for the area <br />was recorded at 28.6 feet. (ALL TRANSDUCER READINGS ARE IN FEET OF HYDROSTATIC HEAD) <br />Insp <br />Inspected the under drain area both the South under drain and the 4 inch pipes were discharging, the <br />discharge was being pumped back onto the leach pad. No major problem noted. Both lower ponds and <br />surface water diversion collection sumps were holding some water from the recent rainfall, but were not <br />discharging. <br />I & E Contact Address <br />NAME: MR.
