ORIGINAL - POBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M_leRl_nn7 <br />INSPECTION DATE: 'i/~1/nx INSPECTOROS INITIALS: ~h <br />OBSERVATION <br />The Division conducted a monitoring inspection of the site as part of sites to be inspected during the fiscal years <br />2005 ar~d 2006. The site is a large sand and gravel operation covering 609 acres with a maximum area to be <br />affected at any given time not to exceed 160 acres. <br />The site was active at the time of the inspection. Mr. Rusty Cochran and Ms Tamara Stevens were of Lafarge <br />were present at the time of the inspection. The entire permit area was driven and inspected. Permit area is <br />fenced, and permit sign was properly placed along the access to the property. There was one crusher and four <br />screens active at the time of the inspection. According to Rusty, approximately 80 acres has been reclaimed <br />along the access road to the property. The reclaimed acres appeared stable and vegetated. <br />Large quantities of processed material were noted near the active pit area along the south side. High walls <br />extending North to south measuring approximately 20 to 30 feet in length anywhere between 50 to 300 feet <br />approximately, were noted. High walls were stable. No major sign of slope failures noted. <br />The setlling ponds for the wash fines were almost full and operator might need to move the ponds when the <br />expansion continues. Three was a conveyer system that according to Rusty measuring 2600 feet that is being <br />utilized for the operation. When the site was last inspected in 2000, the observation was made that the operator <br />was almost reaching the maximum allowable disturbance of 160 acres and for the operator to consider <br />submitting a TR to increase the maximum affected acreage or request have a portion of the reclaimed slopes <br />released. Without taking actual measurements it appears the operator might have reached the Maximum <br />disturbance allowed under the permit. In order to avoid being in violation of the approved permit condition the <br />operator agreed to submit a Technical Revision to increase the permit affected acres to 250 acres. The Division <br />also requested the operator provide in the annual report a survey of the total affected acres. Even though a <br />map is provided with the annual report, the acreage quantity is not included. Operator agreed to provide a <br />survey of the affected acreage when the TR is submitted.
