MINE ID # OR PROSPECTING ID #: M-1993-030 <br />INSPECTION DATE: 5/24/2006 <br />(Page 2) <br />OBSERVATIONS <br />INSPECTOR'S INITIALS: CMR <br />The Crystal Jack mine was inspected at the same time as the pre-operational inspection for the <br />Colorado Calumet Mining. The Crystal Jack is immediately north and adjacent to the Colorado <br />Calumet, and is located off of Forest Road 752. The Mine is located in the Crystal Creek <br />drainage approximately 8 miles north of Lake George. <br />Chris Rowe conducted the inspection and Joe Dorris was present as the operator's representative. <br />In April 2006 the U.S. Forest Service competed a draft Environmental Assessment (EA) on the <br />Crystal jack along with the Glacier Peak, and Colorado Calumet mines (Environmental Assessment <br />Dorris Mining Operations April 2006). The Operator and the Forest Service will use this EA to <br />examine proposed environmental impacts from the mine operations. The EA indicates that future <br />mine impacts from the Crystal Jack will be limited to approximately 0.36 acres per year. <br />The site did not have any open excavations at the time for the inspection. The previous excavations <br />had all be regarded and re-vegetated. The operator appears to be following a concurrent <br />reclamation approach in accordance with the mining and reclamation plan. <br />One concern came up during the review of the permit documents; the maps of the mine site all cover <br />the load claim area of 206 acres. There is not a map that shows the aerial extent of the 2.0 permitted <br />mine area. Nor was there a mine operation or reclamation map included in the application. It would <br />be very useful to the Division if the Operator could prove any of the above-mentioned maps to the <br />Division. <br />According to the EA the Forest Service holds a bond for the Crystal Jack mine in the amount of <br />$6,600. The Division holds a $100 band for the mine.
