~.
(Page 2)
MINE ID # OR PROSPECTING ID # M-86-140
INSPECTION DATE May 29, 1997 INSPECTOR'S INITIALS TAS
1. A site inspection was made of the Wright Pit, located 3.8 miles west of Jefferson along
Colorado Highway 285.
2. The Wright Pit is permitted for 9.9 acres of disturbance with a financial warranty of
$4,500 in the form of a Certificate of Deposit.
3. There was no entrance sign as required by Rule 3.1.12. This requirement was explained
to Mr. Wright and he agreed to post an entrance sign with the appropriate information within
30 days. This item is considered to be a problem that requires specific corrective actions.
Please gee page 3 for details.
4. The site is currently active with approximately 8.3 acres disturbed. There is
approximately 5 acres of pit disturbance and 3.3 acres of processing/stockpile area. The
operator has done a good job of grading the pit side slopes to a 3:1 slope as specified in
the reclamation plan. However, there still revains approximately 1,000 cubic yards of finish
grading to be done in the west end of the pit as well as 4,400 cubic yards of rough grading
to be completed along the southern portion of the pit. This inspector has calculated the
reclamation cost for the current disturbance to be $6,500 (please see attached costs).
Therefore, the operator will need to either supply an additional $2,000 financial warranty
within 60 days or perform reclamation to the extent that the remaining reclamation liability
is $4,500 or less. This item is considered to be a problem that requires specific corrective
actions. Please see page 3 for details.
5. This inspector did measure 1.6 acres of undisturbed rangeland at the west end of the
permit area. Mr. Wright indicated that he would like to "swap" that acreage for 1.6 acres
of land to be mined immediately east of the permit boundary. In order to permit this change
the operator will need to complete the following:
a. Submit a request for financial warranty release for the 1.6 acres to the west in
accordance with Rule 4.16, see attached copy.
b. Following approval of the financial warranty release for the 1.6 acres, submit an
application for a technical revision to the permit to alter the configuration of the
existing 9.9 acre permit boundary. The revision request should include a map which
depicts both the current and the desired permit boundary.
c. In addition to submittal of the technical revision application, you will need to
complete the public notice requirements for a permit amendment as listed in Rule 1.6,
see attached copy. You will be required to resubmit an application for a permit
amendment if any objections are received.
i & E Contact Address
NAME Merril Wright
OPERATOR Universal Peat, Sand & Gravel
STREET P.O.
