The operator should submit a map showing the areas of present disturbance <br />as compared with the approved plan and snap. Such map should also include <br />affected acreages. An amendment to the pennit will be necessary if additional <br />acreage (beyond the 14 acres approved) is desired. <br />'2. .The operator should provide an explanation of the operations in the stream <br />and a reason why such activities are occurriny outside the permit boundaries. <br />IVo such activity may occur under the terms of the approved permit. <br />3. According to the approved permit, enough silt must be salvaged from the <br />settling ponds in order to provide a o-12" soil depth far areas to be <br />revegetated. The operator should indicate what measures are being taken to <br />comply with this provision. <br />4. Surety on this operation is now over b years old (in terms of adequacy). <br />The operator should update reclamation costs by providing an estimate of <br />perfonniny the reclamation as outlined in the reclamation plan. The Board <br />will then act on the matter for the purposes of adjusting surety.
