(Page 2)

~.~.
MINE ID # OR PROSPECTING ID # M-2001-022
INSPECTION DATE 5/29/2001 INSPECTOR'S INITIALS TAS
OBSERVATIONS
1. A pre-operation inspection was conducted of the Green/Croissant Property. Present during
the inspection was Steve Fancher of Loveland Ready Mix Concrete, Inc.
2. The primary purpose of the inspection was to verify the location of water features
and permanent man-made structures as noted on the Mine Plan Map and as discussed in the
permit application. The location of gas pipelines and irrigation return flow ditches was
noted. Mr. Fancher noted that the gas well and pipeline located between Cells 1 and 2 is
abandoned and may be mined through, following written approval from the owner of the
structure. Also discussed was the possibility of needing to relocate any irrigation return
flow ditches. Those owned by parties other than the applicant will require written approval
from the owner prior to relocation.
3. The majority of the site is a sub-irrigated meadow with pockets of wetland vegetation.
This inspector noted a large presence of rushes and some carex throughout the meadow. The
applicant is working through the process of determining whether a U.S. Army Corps of
Engineers 409 Permit will be needed, and also whether there will be wetlands enhancement,
creation, or mitigation within the permit area.
4. The Big Thompson River, immediately east of the Cell 4 area was inspected and
photographed. The applicant has proposed to mine within 110 feet within the channel of the
Big Thompson River. This inspector discussed with Mr. Fancher the Division's concerns for
flood control/mitigation within the context of the Construction Material Rules & Regulations.
Various options for resolving the Division's concerns were reviewed with Mr. Fancher.
I & E Contact Address
~ CE
NAME Steve Fancher ~ BL
OPERATOR Loveland Readv Mix Concrete, Inc. ~ FS
STREET P.O.
