(Page 2) <br />MINE ID # OR PROSPECTING ID #: M_1985-0? <br />INSPECTION DATE: 2/I'~/nx INSPECTORS INITIALS: ~1' <br />OBSERVATIONS <br />l . This inspection was performed as a follow up to an inspection of the site conducted on June 28, 2006, <br />during which numerous problems were cited. Present at the inspection was Kate Pickford, DMG. <br />Weather at the time of the inspection was clear, calm, and approximately 70 degrees. <br />2. During the previous inspection it was observed that there was no sign at the entrance to the site; <br />numerous 55-gallon drums and 5-gallon buckets containing oil and other unknown materials were at the <br />site, and some of the material contained in the containers had leaked; and there are multiple above <br />ground storage tanks that have no containment structures. Additionally, during the previous inspection, <br />it was observed that there is a large amount of equipment and other items stored at the site. <br />3. During this inspection, it was observed that a sign has been posted at the mine entrance and the 55 <br />gallon drums and 5 gallon buckets have been removed. Additionally, the soil where the contents of <br />some of the drums and buckets had spilled has been overturned. The containment structures below the <br />above ground storage tanks have not, however, been installed. The remainder of the items previously <br />at the site are still present, with the exception of one of the inoperative trucks, which was removed. <br />4. Since the time of the previous inspection, Joe Dodge, the manager of the site, contacted the Division <br />regarding the previous inspection. Mr. Dodge stated that the items stored at the site are all essential to <br />the operation and must remain onsite. <br />5. The problems cited in the previous report have been resolved, with the exception of the lack of <br />containment structures below the above ground storage tanks. As stated in the previous report, the <br />Operator must submit a technical revision to the Division stating that the tanks will remain empty until <br />containment structures are constructed below the tanks. The technical revision must include designs <br />that indicate the containment structures will be adequate to contain a spill in the amount of the tank <br />capacity. Additionally, the Operator must submit the Spill Prevention, Control and Countermeasure <br />plan for the site to the Division. The above, required information must be submitted to the Division for <br />review within sixty (60) days of receipt of this report, or by November 25, 2006. <br />4.
