Locations as of January 17, 2007
Look in the bookshelf along the north wall for an Environmental Management Documents
(EMD) binder. A 4-drawer file cabinet is located against the east wall.
1) A copy of the permit is filed in the Environmental Management Documents (EMD) binder,
Tab 2. It expires May 23, 2008.
2) The permit application w/revisions is on shelves in the south wall bookshelves. It consists of
20 Volumes: I, IA, II (and sub volumes for Exhibits 1 through 49), III (Figures), IV and IVA (Tables
1 through 69), Volume V (Maps 1 through 41 A), and Fish Creek tipple Volumes VII, VIII, and
IX.
3) The Findings Document for the latest Permit Renewal is filed in the EMD (Tab 2) The current
one is for Renewal 4 and is dated December 30, 2003.
4) The insurance certificate is filed in a file cabinet drawer labeled C-82-056. Certificates
expire on 10/Ol of the applicable year.
5) Copies of bond documents are in the top drawer labeled DMG 82-056. The current
insurance document is Travelers Casualty and Surety Company No. 1 0429 4 1 44 in the
amount of $7,251,822 (subject fo change with revisions).
6) Air emissions (construction) permit #93R01204 issued April 10, 2001 is on file in the EMD, Tab
1. It has no expiration date.
7) A copy of the Routt County Special Use Permit (SUP) 00-344, changed to SUP 93-233 valid
until September 1, 2003 and extended to 07-1 1-10, is in the EMD at Tab 5.
8) UG Mining Landowner Notifications are filed in the 82-056 drawer, in a file labeled "Land
Ownership Notifications".
9) Subsidence Monitoring Reports are filed in the 82-056 drawer, in a file labeled "Current
Subsidence Reports".
10) Subsidence monitoring data is stored in a binder, located in the north shelves.
1 1) There are no variances on this permit with regard to Rules 2.06.2(1) (Experimental
Practices Mining), 2.06.3(4) (Mountaintop Removal Mining, or 2.06.5(3) (Steep Slope
Mining). Variances were provided in other permit PAP under Rule 2.06.7(5) for Delay in
Contemporaneous Reclamation of past surface mining activities which accommodate
current underground entries and surface support activities. These variances need to be
collated into the current PAP (see PAP, page 2.06-3. Unless soon remedied, this inclusion
will be an item of the 2005 midterm review.
12) Annual Reclamation Reports are due by February 15 of the following year [Rule
2.04.13(1) (no other such date agreed uponJ]. ARRs are on file in the 82-056 drawer of the
file cabinet in a folder labeled "Annual Reclamation Reports".
