(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1980-131 <br />INSPECTION DATE 4/12/2005 <br />INSPECTOR'S INITIALS SSS <br />This inspection was conducted to observe and verify diesel spill remedial actions taken by Elam Construction, Inc., operating <br />this site which is leased from Lafarge West, tnc., the permit holder. <br />On April 2, 2005 Elam Construction ,Inc. reported to DMG that a spill of an estimated 650 gallons of diesel fuel had occurred <br />aver the weekend of March 26-27, 2005 and was discovered on the morning of March 28`". The report to DMG was delayed <br />due to a mis-communication between Elam and Lafarge personnel. The report indicates that the fuel leaked through a hole in <br />a supply line thatwas apparently chewed by a rodent. The tank was located in spill containment, but the nature of the leak and <br />the stream of fuel spraying from the hole in the supply line caused an estimated 85 percent of the fuel to be lost outside the <br />containment structure. Upon discovery of the leak, the supplyvalvewas closed and measures were immediately implemented <br />to contain the discharge, including placement of an absorbent boom in the adjacent stream channel. <br />To facilitate clean-up, the asphalt tank and mixing drum had to tie relocated to allow for excavation of diesel contaminated soil <br />beneath those components and around other tanks, conveyors, and equipment trailers. <br />Upori arrival at the site for this inspection, it was noted that soil had been excavated approximately 1.5 to 2.5 feet deep around <br />the area of4he spill. Excavated material was temporarily placed;in.a bermed area lined with black plastic adjacent to ther;:_ ,:- <br />asphalt plant. Mr. Lymah indicated that Elam was proposing to decontaminate the soil by.running it throughthe asphalt plant c..:~, <br />burner to burn off the diesel The area was visually inspected and'randomly checked for smell of diesel. in the soil: A few areas.-, <br />-=were~no4ed wFiei•e diesetwss still in the sdil. It appeared that the initiaLexsavation was deep enough to collect and remove the - <br />contaminated soil effectively preventing it from seeping into the water table. <br />The water surface of the adjacent stream channel and the adjacent pond were inspected and no visible sheen was observed, - • <br />nor any residue on the rock edges at water surface. ~ I <br />The operator has contacted'the Colorado Department of Public Health and EnwronmentQGpPHE) regarding the spill and wall <br />clean the site up per their directions and requirements. Soil samples.were collected by the operator in several locations within .:. <br />the area where the contaminated soil was excavated. The samples were sent~to a lab foranalysis. The results indicated all ; <br />samples were below the 500ppm limit for total petroleum hydrocarbons. Though the samples were within the limits set by <br />CDPHE indicating that no additional remediation be required, one sample was close to 500ppm for tph so Elam indicated that <br />an additional 6-8 inches of material was removed. - .. . _ __ <br />,r?.,. }.
