(Page 3) • <br />MINE ID # OR PROSPECTING ID # M-87-038 <br />INSPECTION DATE 02/20/98 INSPECTOR'S INITIALS RCO <br />There is no existing mining-related disturbance in the smaller amendment area, though <br />excavation has encroached fairly closely. The operator indicated that eventually the two <br />large, existing pits may be connected by mining the strip of material which separates them, <br />so the operator is including this small area in the amendment to facilitate this plan. <br />II. Monitoring inspection of the existing permit area <br />The past operation has produced a large pond, which is currently filled and ice-covered. <br />Minor future operations will occur on the west side, as Phases 2-4, which will move the banks <br />toward the west. There is now a dirt road along the west side, approximately on the permit <br />boundary. The operator should take great care to ensure that no disturbance exceeds the <br />permit boundary. <br />There is a large processing and batch plant west of the operator's office and shop buildings. <br />This area was once part of the permit, but has been deleted. The margin of the large pond <br />which includes Phases 1-4 is now permanent on the south and east sides. No further <br />enlargement will occur on those sides, and final reclamation should be finished there. As <br />mentioned above, the north margin of the pond is at the location approved in the current <br />mining plan, and if the operator wishes to mine further there, and connect the two ponds, a <br />revision to the approved plan must be submitted for review. <br />Phases 5-7, in the north pit, are well underway. Mining in Phase 5 is almost complete. Most <br />all the minable deposit is removed, and all that remains to perform is minor grading of <br />reject material presently on the west side of the floor. Some bank regrading on the <br />northwest has occurred. Phase 6 is mined down about halfway, and some operations presently <br />occur on that bench. Phase 7 has been stripped and extensive product stockpiling and <br />crushing plant operations are presently carried out there. All crushing equipment is <br />portable, and the fuel storage tank includes good secondary containment. <br />All mining, processing and stockpiling appear to be outside the necessary 60-foot setback <br />radius from the gas well on the south edge of Phases 6-7. There is a 20-foot highwall, <br />however, which seems to be at or near the 60-foot setback, which is between vertical and 1:1, <br />which should be backfilled to buttress it against failure. This should be done soon, unless <br />the operator can show evidence of abandonment of the well. Associated with the well are <br />three buried pipelines radiating from the well: one (which is depicted on the maps) leading <br />away to the south through the processing and office area, another leading to the west in the <br />unmined material separating the two main pits, and the last leading to the north through <br />Phase 7. The last one was found by accident during the present operator's mining. Several <br />sections are exposed now, but .there is no apparent damage. This will surely have to be <br />rerouted before mining becomes more extensive in that phase, because as the operator stated, <br />this northern line is now the only one active. Several surface structures depicted on the <br />maps as near the wellhead have been removed. <br />The entire north edge of the pit is mined down to about the final depth of 15 to 20 feet. <br />A trench to facilitate dewatering the pit extends the length of the toe of the highwall. <br />Stripped topsoil is located in a stable berm across the north edge of the pit and a smaller <br />berm was observed across the east edge. (These stockpiles should be seeded to grass to <br />protect them during time in storage. The operator stated that the pile along the east <br />especially will be seeded in 1998, since it will be there for a longer time.) Dewate ring is <br />carried out by pumping from the sumps in Phase 5 (with a diesel pump located on the unmined <br />strip between the pits) into the south pond. There are a series of drain trenches across the <br />floor of Phase 5 leading toward its south edge. <br />The wetland/wildlife area, mentioned above, was observed.
