• (Page 7) • <br /> MINE ID # OR PROSPECTING ID #: M-1978-352 <br /> INSPECTION DATE: in/19/no INSPECTOR'S INITIALS: ('I K <br /> OBSERVATIONS <br /> The following observations were made during a routine inspection of this 52 acre 112 <br /> site . Excavation of the site has been completed, and the activities have moved into <br /> the reclamation phase. <br /> 1) The sign at the entrance to the site has faded into illegibility. This must be <br /> remedied within 30 days . (P81) <br /> 2) Topsoiling and grading of the site has been completed, with most slopes varying <br /> between 2H: 1V and 3H: 1V. While the topsoil is not top quality, it is thick enough, <br /> and mulch and soil amendments are present on-site to improve the soil . <br /> 3) The first round of fall seeding has been completed, and hopefully there will be <br /> some progress toward re-vegetation by next spring. <br /> 4) The berm along the north boundary, separating the site from the irrigation ditch, <br /> had been preserved, along with the 50' setback from the River. <br /> 5) No exposed groundwater or pooling surface water was present at the time of the <br /> inspection. The one runoff ditch cutting across the slope from the upper bench to <br /> the lower area of the pit has been sufficiently lined with riprap to prevent <br /> erosion. <br /> 6) All power poles appeared to be stable, and possess a 10 ' setback from the edge of <br /> the slope. The "island" telephone pole left in the middle of the site retains <br /> accessibility for its maintenance by the power company. <br /> 7) All equipment and structures remaining on the site appeared to be portable. <br /> No other problems were noted at the site at this time. A re-calculation of the bond <br /> will be done within 60 days, and the Operator will be informed if there is any <br /> reduction to the bond due to the completed reclamation tasks . <br /> I & E Contact Address Gypsum, CO 81637 <br /> NAME: John C.
