<br />MINE ID # OR PROSPECTING ID #: M-iaBn-~aa <br />INSPECTION DATE: i ~-~n-nn INSPECTORS INITIALS: ~lC- <br />OBSERVATIONS <br />The Division conducted a monitoring inspection of the site on 11-20-00, while in the area for a quarterly <br />meeting with local officials. The main purpose of the site visit was to attend a quarterly meeting with local <br />officials regarding the operation and be briefed by CC&V about the upcoming wnstruction activities that <br />will be taking place on site. Present during the meeting were Jane Manon, Jean Garren, Bill Johnson and <br />Randy Swepson from Teller County and Mayor Jim Watson of Victor. Mr. Phil Barnes of CC&V gave a brief <br />presentation about the project for those local representatives that were new to the area and updates of the <br />upcoming construction activities. The local agency representatives asked some questions regarding the <br />operation, which the company addressed. <br />Prior to the meeting, a physical inspection of the site was conducted. In anticipation of the highway <br />relocation, topsoil pile from the main storage area North East of the ARD building was being moved as <br />approved in Amendment #8. Operator has constructed a long silt face orienting North to South along the <br />topsoil relocation area. A portable cement batch plant for the highway bridge construction was in place <br />South of the Topsoil relocation area. The area where the external pond will be constructed was being <br />stripped of topsoil. The road that will be used to construct the bridge pier has been cut and no problem <br />noted. The West Side Bridge pier forms were in place at the time of the inspection. Inspected the leach pad <br />area. According to Mr. Barnes, all the primary header pipes have been replaced with steel pipes. While in <br />the Phase II area, the following readings were taken from the Phase I and II high volume and low volume <br />pressure transducers. Low Volume pump # 1 measured at .09 feet of head, # 2 measured at .11. High <br />volume pump # 4 was not functioning, Pump # 5 measured at 27.4, # 6 measured at 27.6 (all <br />measurements are in feet of head). <br />Inspected the substation relocation area final grading of the area appeared to be in place. No problem <br />noted. Inspected the crusher area, it was down, according to Phil, it has been down the past few days with <br />some minor problems. Run off mine material was being loaded onto the pad with lime application in situ. <br />Inspected the main Cresson pit area, no problem noted. Inspected the Squaw Gulch overburden storage <br />area, it was active at the time of the inspection.
