(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-rQa'+-ntL- <br />INSPECTION DATE; ~-~n_nd INSPECTORS INITIALS: JL>-_ <br />OBSERVATIONS <br />Completed a partial inspection of fhts operation fo review mine activities wffh Mr. Tony Waldron, Division <br />Environmental Protection Specialist, since Mr. Waldron is expected to take over the site (as lead specialist) in <br />the future. <br />This is a 260 acre 112 Regular permit located about 2 miles north of Fairplay, east of Highway 9. The affected <br />land area is limited to 79.5 acres. An adequate mine ID sign has been placed at the enfrance of the permit <br />boundary and the operator is in the process of re-Installing affected land boundary markers (this was noted as a <br />problem during the May, 2004 Inspection). <br />Walked around the affected land area. Used a GPS to determine the approximate acreage of the excavated pit <br />(since it is resMcted to 8 acres). The excavated portion of the pit appears to be within the 8-acre Iimff based on <br />GPS measurements. The operator intends fo begin baekfliting the pit fn the near future (actually started <br />backftlling on July 24, 2004). <br />Found a few yellow toadtiax plants and a few Oxeye daisy plants (both are on the noxious weed list) in the <br />affected land area. Mr. Clare was Instructed to eradicate the weeds before they become a severe problem. <br />Also, the operator needs to submit a weed confrol plan describing how the noxious plants will be kept out of the <br />permit area. <br />The main components of the reclamation plan are dirt work (dozing and grading), topsoil replacement, <br />reseeding (includes planting tree seedlings) and removal of all the used equipment, hash and refuse. <br />Most of the used equipment is stored in the north cenfral part of the affected land area. Currently, more junk is <br />accumulating in the storage area. Since this site Is not permitted as a landfill the operator was instnrcted to <br />remove all of the junk and used equipment from the site. This was noted as a problem on page 3 of the May, <br />2004 Inspection report. The operator needs to complete the corrective action by August 10, 2004. Failure to <br />complete this corrective action, or any other corrective action, could, resuff in the issuance of a possible notice <br />of violation. <br />There have been several personnel changes at this site during the last year. The name of the contact person has <br />not been revised since Mr. Jon Harms was manager of the site.
