(Page 2)

MINE ID # OR PROSPECTING ID #: na-i 97Fi-nn9HR
INSPECTION DATE: in-lR-nn INSPECTOR'S INITIALS: ~-
OBSERVATIONS
The Division conducted a monitoring inspection of the site on 10-18-00. The site is a large 112-granite
quarry operation located approximately 16 miles South West of Colorado Springs. Present during the
inspection were Messieurs Mark Heffner and Scott Davis of Centennial material. The site was active at the
time of the inspection. The pit floor was holding some water from the recent storms. The quarry is mined in
benches. There were six benches noted along the south East Side of the pit area. High walls were stable and
no major failure surtaces noted. The pit floor area was holding some processed material along the north
East Side outside of the ponded area, no problem noted. All the available top soil remow:d from the area
that is not used to reclaim the benches were properly stockpiled and protected along the perimeter of the
pit area. The site was amended in 1997 and financial warranty adjusted at that time. The existing warranty
is deemed adequate at this time. The storm water protection stream berm along the access road appeared
to be in good repair. The small storm water detention basin located along the South East portion of the
quarry area was holding some water and appeared to be functioning as designed. Permit sign and boundary
markers were properly displayed.
I & E Contact Address
NAME: Scott Davis
OPERATOR: Centennial Materials Inc
STREET: 2b35 Del a Drive
CITY/STATE/ZIP: Colorado SRring5,C0 Rn91n
James Stevens
cc: ^ CE
^ BL
^ FS
^ HW
^ HMWMD (CH)
^ SE
^ WQCD (CH)
^ OTHER
