Cormrents also describe any errforcement actions taken during the inspection and the fads or <br />evidence atpportinp the enforrx~vsent action. <br />This was a partial inspection conducted by Janet Binns and Byron Walker of CDMG. The mine is <br />reclaimed and the permit has been revoked No facilities remain an site. No personnel are connected <br />with this mine site. <br />The focus of this arspection was fb foIIow up on the two slides noted in Division of Minerals and <br />Geology's May 11,1998 inspection. Mr. Walker assassed the sate in terms of engineering stability. <br />Mr. Walker's interoffice memo accompanies this report. <br />The slide West of Pit 4 did not appear to have enlarged since the May 11,1998 inspection. It appeared <br />that the ran out of the side had reached Grassy Creek, but had not blocked the creek. Grassy Creek <br />was Sowing unimpeded at the time of this inspection. the dimensions of the mndtlow are visually <br />estimated and paced at 40 R Wide by appn+wc. 120 R Long, with the toe of the slide spreading to ~ 90 <br />R wide. Water ie flowing out ofthe slope ofthe slide at appmx Less than 3 gallons per minute. <br />Investigation of Pond 4 at the base of Pit 4 Ends the riser undercut and piping along riser to have <br />occurred. Fipung is not apparent on ontslope of embankment Jnet below the pond embankment on the <br />west side of the pond ontalope vegetation consists of very dense oak brush. 'Iltorongh investigation of <br />the slope below the pond is }mrited by the dense vegetation The head of the scarp below the pond was <br />accessed. Head scarp originates approx 70-90 R Below the pond embankment. Gmliying in elope <br />above the ecazp and bekrw the pond embaalanent was observed Slide head scarp is appmx 5 R in <br />heigjrt. A sediment delta apper+enfly from the run onY of the flexible pond riser sleeve has accnmuhrted <br />at this loc:atian. A spuing fi+om the hilWope ahro exln'bi>e itself here. Visually it appears that nahual <br />causes contabute to the formation of the slide, however, contnbutioa of concenhated water from Pond 4 <br />may add to the instabr7ity of the slope. <br />Hairpin tmn on the acx;ess road to Pit 4 cartinnes 1p cut. <br />Piping of the ditch along access road to Pit 2/3 has occuaed.
