• (Page 2) • <br />!'ENE ID # OR PROSPECTING ID # M-94-117 <br />INSP~~'T ION DATE 4-13-98 INSPECTOR'S INITIALS ACS <br />OBSERVATIONS <br />This inspection was conducted to assess compliance with the approved water management plan <br />for the Gold Hill Mill. This plan is designed to maintain the site in a zero-discharge <br />configuration. There was no representative of Colina Oro Molino, Inc. present for the <br />inspection. Ernie Simmons representing ITEC Mineral was present during the inspection. <br />Freeboard in the tailings pond was measured at 19 inches. The spray evaporative system has <br />been repaired and upgraded and was operational at the time of the inspection. The spray <br />evaporation system must continue to be operated daily until the water level in the pond is <br />at the lowest practical level for the recirculating pump intake port. Several sprinklers in <br />the tailing pond were spraying onto the unlined portion of the dam crest. The approved <br />permit requires that in a calm wind condition, the sprinklers must be set to confine the <br />spray to the lined area. The sprinklers were properly adjusted prior to the end of this <br />inspection. <br />The storm water management berm below the ore loading ramp into the mill was inspected and <br />found to be in good condition. The small sediment pond at the low end of the mill site, <br />between the tailing pond and the mill building, was inspected and found to be functioning <br />properly. There was a small amount of standing water in the sediment pond at the time of <br />this: inspection. The upland diversion ditches above the tailing pond were inspected and <br />found to be free of obstructions. The small borrow pit above the tailing pond was noted to <br />have: about two feet of standing water in the bottom of the pit. <br />The Hazel-A adit was inspected. The secondary sump pump (first pump inby of the portal) was <br />found to be working properly. The primary sump pump (first pump outby of the wooden <br />bulkhead) had pumped the area behind the coffer dam completely dry. The pump was running and <br />suc}:ing air because the float switch was stuck. The float switch was freed, and the pump <br />shut: down. A new float triggered pump has been installed behind the wooden bulkhead. <br />Freeboard behind the bulkhead was 28 inches at the time of this inspection. <br />The log book for the required daily site work is now being kept in the mill building. This <br />log book was inspected, and no significant problems noted. The operator is advised to make <br />the following minimum entries into the required daily log: i) Date and name of person making <br />the entry; 2) Condition of each pump - recirculating pump in the pond, pump behind Hazel-A <br />bul}:head, primary Hazel-A sump pump, secondary Hazel-A sump pump; 3) Approximate number of <br />hours that the spray evaporative system was operated; 4)condition of all pipes and fitting, <br />both within the pond and running from the Hazel-A to the mill; S) Freeboard in the pond and <br />freeboard behind the Hazel-A bulkhead; 6) Approximate temperature and general weather <br />condition; 7) Any observations of note. <br />Based on the observations made during this inspection, the Gold Hill Mill water management <br />system has been repaired and is now operating properly with the exception of the land <br />app:Lication system. The problems noted in the Division's 3-31-98 inspection report have been <br />resolved. <br />
