The operator has failed to properly handle and/or <br />dispose of a potentially hazardous substance so as to <br />protect the surface drainage system from being <br />polluted. Specifically, a large liquid storage tank <br />was located on the mine site which did not have <br />containment berms constructed around it and had leaked <br />or discharged a organic polymer type substance onto the <br />ground surrounding the tank. <br />Corrective Actions are to: <br />1) Positively identify the substance chat was being <br />stored in the tank and submit an MSDS sheet for <br />this material to the Division. <br />2) Clean-up the spilled material and dispose of it in <br />an approved manner, such as using it for its <br />intended use or having it removed to a facility <br />permitted to receive such waste. <br />3) Provide evidence tho the Division that the spill <br />has been properly disposed of such as photographs <br />and receipts for disposal. <br />03/18/99 <br />PB or PV: PV <br />Topic(s): HB, <br />MP, AT <br />
