(Page 2) <br />i <br />MINE ID # OR PROSPECTING ID # <br />~LNSPECTION DATE 2/8/99 <br />M-77-376 <br />OBSERVATIONS <br /> <br />INSPECTOR'S INITIALS AJW/JCS <br />This was a follow-up monitoring inspection conducted by Tony Waldron and Jim Stevens of DMG. <br />The operator was contacted prior to the inspection and chose not to be present. The site is <br />located just west of Canon City approximately one mile west of Skyline Drive. <br />The site was inspected in March of 1998 and two problems were identified along with <br />corrective actions. As of this inspection, neither of the problems have been corrected and <br />gas a result, they are both being cited as possible violations along with the necessary <br />corrective actions. "Reason to believe" letters are also being sent out which will identify <br />times for the next available Board Meeting and the possible fine schedule. <br />The first citation is for failure to post a mine identification sign at the mine entrance and <br />failure to clearly mark the affected area boundaries. The mine is situated in an area with <br />,extensive pre-law mining disturbance. Therefore, it is essential that the boundaries be <br />snacked so that these disturbances can be properly tracked. In addition, the Canon City <br />4dminisCrator has expressed some concern about the west boundary in particular, as they own <br />,the property that abuts up to the mine on the west. Apparently the city is concerned that <br />mining has encroached onto their property, however, until the property boundaries are clearly <br />marked it will not be possible to determine this. Observations made during this inspection <br />iwould seem to indicate that the area along the property boundary has not been mined, however, <br />the area may have been affected by mining in that settling and slumping has occurred in the <br />general vicinity of the property boundary. This appears to have occurred some time ago and <br />may have been the result of pre-law mining activities. As stated previously, until the <br />affected area boundary is clearly marked, it is not possible to determine exactly where this <br />3isturbed area lies. <br />The second citation is for failure to properly handle or dispose of potentially hazardous <br />material. Specifically, there is a large petroleum type storage tank that has leaked a <br />portion of its contents out onto the ground. The operator needs to positively identify this <br />substance and clean up and properly dispose of the spilled material. Both of these items are <br />'listed on page three of this report along with appropriate corrective actions and dates. <br />Once the affected area boundary is clearly marked, the Division will conduct another <br />'inspection to determine whether mining has caused off site damage and if so, was it pre-law <br />or the result of more recent mining activity. <br />T & E Contact Address <br />NAME Canyon Dolomite Quarry <br />OPERATOR Garv W.
