(Page 2) <br />MINE ID # OR PROSPECTING ID #: ~-79R~-add <br />INSPECTION DATE: 8-id-03 INSPECTOROS INITIALS: --BK- <br />OBSERVATIONS <br />The Division conducted a monitoring and construction progress Inspection of the Phase I, II, III AND IV areas of <br />the leach pad on 8-14-03. Present during the Inspection were Mark Tidquist and Phil Barnes of CC&V. The <br />physical inspection began around the Phase IV pregnant solution collection areas. <br />While in the area collected the following readings from the High and Low Volume Solution Collection <br />Transducers for Phase IV. High Volume Transducer # 1 was not functioning at the time of the inspection, the <br />pump was on, once all the power transfer is complete, it should be functioning as designed # 2 was at 25.7 feet <br />of hydrostatic head. Low Volume transducer # 1 was at 6.7 and # 2 was at 3.2 (all readings are in inches of <br />hydrostatic head) <br />Inspected the Phase I AND II Pregnant solution collection areas and recorded the following readings from fhe <br />High and Low volume solution collection areas. <br />Phase I HIGH VOLUME SOLUTION COLLECTION TRANSDUCER # 1 WAS recorded at 33.4# 2 was at 40.4 and # 3 was <br />at 39.8. Low volume transducer # 1 was recorded at .22 # 2 was at .60. Piezometer in the area was recorded at <br />43.8 feet. <br />PHASfit HIGH VOLiiMf SOLtTION COLLECTION TRANSDUCER # 4 WAS RECORDED AT 33.1 # 2 WAS AT 27.2 AND #6 <br />WAS AT 33.0. LOW VOLUME TRANSDUCER # 1 WAS AT .16 AND # WAS AT .34 (ALL READINGS ARE IN FEET OF <br />HYDROSTATIC HEAD) Piezometer for the pond was recorded at 33.9 feet. <br />CCBV had reworked some erosion features noted below the external pond and placed riprap, to protect it from <br />further damage in the future and it appeared to be In good repair. <br />Inspected the external pond, It was holding water from the recent storms, while in the are recorded the following <br />readings from the Low Volume solution collection transducers # 1 was at 8.6 and # 2 was at 11.4 readings are in <br />inches of hydrostatic head. Earth moving work in anticipation of liner deployment, below the ARD area for the <br />additional containment area, had commenced, no major problem noted. <br />Inspected the under drain area. Both the 4-inch pipe and South under drain were dlscharging at the time of the <br />inspection. The flow was being pumped back onto fhe leach pad. All three lower ponds were still holding a lot <br />of water; the wafer was being pumped back onto the pad at the time of fhe inspection. All the lower ponds and <br />surface water diversion structures were also holding water but were not dlscharging at the time of the <br />inspection.
