L • (Page 2) • <br /> MINE ID # OR PROSPECTING ID M-78-352 <br /> INSPECTION DATE 9-24-96 INSPECTOR'S INITIALS GSC <br /> OBSERVATIONS <br /> This was a follow-up inspection to verify completion of corrective actions conducted in <br /> response to the Division's inspection conducted on 7-29-96. Mr. Claude Gerard of Roaring <br /> Fork Resources was present during the inspection. <br /> Problem 1: There is inadequate spill containment for fuel tanks located near the crusher, <br /> wash plant and scale house. <br /> The following spill containment measures are considered adequate: <br /> Scale house fuel tank - An additional plastic liner was installed beneath the worn liner <br /> already in place. The earthen containment berm was reinforced. <br /> The 10,000 gallon steel fuel tank by the crusher - An earthen spill containment berm lined <br /> with plastic has been installed below this fuel tank. The liner has been covered with sand <br /> to protect it from the sun. <br /> The following spill containment measures are not considered adequate: <br /> Wash Plant Area - An earthen berm had been constructed around the 2,000-4,000 gallon fuel <br /> tank located by the wash plant. The berm is insufficient along one side of the tank and the <br /> remaining walls of the berm are too close to the tank to allow for adequate emergency storage <br /> capacity. This tank may be located too close to the wash plant and conveyors to allow for <br /> sufficient room to construct an adequate berm and it may be necessary to relocate it or <br /> consider an alternate type of spill containment such as concrete. Mr. Gerard requested a two <br /> week extension to empty the remaining fuel in the tank and complete the corrective action. <br /> The Division accepts the operator's request to complete this corrective action and the new <br /> due date for completion is October 9, 1996. Photographic verification is due no later than <br /> 10-16-96. <br /> A 100 gallon waste oil tank and some drums of lubricant are located in the vicinity of the <br /> equipment maintenance area near the wash plant. The tank and all drums on-site need to be <br /> placed in lined storage area(s) with adequate spill containment. This needs to be completed <br /> and photographic verification submitted to the Division no later than 10-16-96.
