iiiiiiiininiiiiii <br />999 <br />COLORADO DIVISION OF MINERALS AND GEOLOGY <br />MINE SAFETY AND TRAINING PROGRAM <br />INSPECTION REPORT <br />Inspection Date: August 4,1997 <br />Name and Location of Operation: Bowie No. 2 Mine, Delta County, Colorado <br />Bowie Resources Ltd. <br />Name of Person In Charge: Bill Bear Jr., Mine Manager <br />Reason for Inspection: Safety Audit and Investigation of Safety Complaints <br />State Inspectors: Bill Davies and Bill York-Feirn <br />~~ <br />On July 22, 1997, the Mine Safety and Training Program received a memorandum from Joe <br />Dudash with the Division's Coal Mine Reclamation Program which detailed several serious <br />concerns at the Bowie No. 2 construction site and a request for assistance. On July 23, a meeting <br />was subsequently held with several Coal Program supervisors and inspectors to discuss these <br />concerns in more detail. This inspection report details the Mine Safety and Training Program <br />response to these concerns. <br />Bowie Resources Ltd. hired a conuactor to conswct 2 miles of access road and a rock face area <br />on the hillside where the portals for the new Bowie No. 2 underground coal mine will be driven. <br />We met Bill Beaz Jr., Mine Manager, at the Bowie No. 1 mine office and discussed these <br />concerns before completing an inspection of the Bowie No. 2 site. The concerns brought to our <br />attention were investigated together with Bill Beaz and the following recommendations were <br />made: <br />(1) A complaint was received by the Coal Program that said that flyrock from the site hit a house <br />located about 2,000 feet from the Bowie No. 2 blast site. We stopped by the house and did not <br />observe any appazent damage on the roof or walls from flyrock. We stood at the site of the blast <br />and looked down at the house which appeazed to be about 2,000 feet away. The blast was <br />delayed to throw the rock to the east, instead of to the south where the house is located. At any <br />rate, it was very difficult to see how rock from such a small shot could have reached the house. It <br />was recommended that everyone should be removed from the area if there is even a remote <br />chance that flyrock could reach the house. The public road (old highway 133) to the south <br />should have been guazded to prevent access from either direction. As of August 6, the conuactor <br />is finished with all blasting on the surface azea of the mine site. If any blasting is needed while <br />driving the portals, it will be located at least 2 miles from any house or public road. <br />(2) We investigated the certification and training records to determine whether the proper <br />certifications and safety training had been completed for all contractors working on the site. <br />
