' • (Page 2) • <br />r MINE ID # OR PROSPECTING ID #M-80-244 <br />INSPECTION DATE 8-18-98_ <br />INSPECTOR'S INITIALS BK/JD_ <br />The Division conducted a monitoring inspection of the area on 8-18-98.Present during the <br />inspection were, Mr. James Dille of the Division and Mr. John Hardaway of CC&VG. The purpose <br />of the inspection was to see the location of the new ground water monitoring wells that were <br />being drilled as replacement to the ones that sheared off and to visually see the location <br />of the proposed haul road realignment as submitted in the Technical Revision. The operator <br />had submitted a technical revision to replace the wells and also adjust the haul road and <br />operating face of the existing leach pad slope from 2.5:1 to 1.6:1. <br />A brief discussion was held with Mr. Hardaway about the reasons why the monitoring wells <br />sheared off. He stated that the shearing was due to the weight of the casing and not due to <br />the weight of overburden being loaded up gradient of these wells. Since these wells were <br />considered up gradient wells, and the Division has more than five quarters worth of data, the <br />Division did not consider it to be a major problem. Because the wells were already covered <br />with overburden, the Division staff could not ascertain the real cause of the shearing and <br />had to accept Mr. Hardaway's explanation. <br />Inspected the area where the new wells were being drilled. According to Mr.Hardaway, the <br />shallow well encountered water at an 197 feet appx. The deep well was at appx 300 feet at the <br />time of the inspection but still did not encounter water. It was suggested by Division staff <br />that water had to be encountered even if the operator has to drill to 400 feet. Mr. Hardaway <br />was informed that the Division will not allow the replacement of two producing wells with <br />only one producing well. Inspected the area where the proposed haul road realignment is to <br />take place.
